Synopsis
B777-200 flight crew reported a glideslope failure during final approach resulted in two go-arounds. The crew was able to land by executing an RNAV approach. The flight crew also reported an altitude deviation during the first go-around.
Narrative
While flying the ILS XR into ZZZ in actual weather and below approximately 1;000 feet the CA stated he had an amber line through his GS annunciation and was also noted on the FO display as well. During the go-around; a Tower directed climb and turn were given. The turn was approximately 90 degrees to the left of the current course and an altitude of 3;000 feet to maintain. The autopilot disconnected at some point during the go-around and the altitude of 3;000 feet was exceeded. The aircraft was then leveled at 3;000 feet on the Tower directed heading and setup for an ILS approach to Runway Y. During this approach to a different runway and just inside the FAF both the CA and FO had an amber line through the LOC. A second missed approach was initiated; followed by RNAV approach to Runway Y followed by a normal landing. After landing we requested the Tower to check with following aircraft if they had any fail indications on the ILS approaches to Runway XR or Y. Other aircraft stated they had no problems. Maintenance was notified and the aircraft was written up. Cause - Equipment failures; late at night; actual weatherSuggestions - Practice Tower directed go-arounds above 1;000 feet AGL with a low altitude level off.
Second reporter narrative
Set up for approach to Runway XR at about 800 feet glide slope failure. Started published go-around. Tower issued a turn 90 degrees to north and 3;000 feet. Autopilot disconnected and while doing Tower issued go-around exceed altitude limit by several hundred feet. Leveled at 3;000 and resumed Tower instructions. Set up for ILS to Runway Y and received LOC failure flag. Followed tower instructions for missed approach and flew a RNAV approach to Runway Y to minimums and landed. Raining and clouds to about 500 feet. No traffic conflict. Tower said no problems with their approaches. Wrote up aircraft and talked to maintenance.Cause - Bad weather; late at night; not published go-around and equipment failures.Suggestions - Go-around practice from 800 feet with turns and hand flown.
