B737-900 Captain reported a flight delay due to fuel management panel configured incorrectly and missing log book entries. The Captain requested maintenance action and resolved the issues.
Narrative
During the First Officer's initial fight deck prep; he discovered that all 6 fuel pump switches were in the on position with the Center Tank Fuel quantity at zero. Because this is a limitation exceedance; and the FOM states that any limitation exceedance suspends the airworthiness of the aircraft; he entered the discrepancy in the logbook. The maintenance technician arrived at the aircraft; took the logbook; and left the airplane. Sometime later; the technician returned with a signed off logbook. I asked what he did; and he responded that he turned the center tank fuel pumps on (now with more than 1;000 lbs of fuel in the Center Tank) and checked to see if the LOW PRESSURE lights functioned properly. They did; so he signed it off. As I was reviewing the signed off discrepancy; I noticed on the previous page the signature; date and employee number was missing from the oil service block. I entered this discrepancy in the logbook; and again; a maintenance technician came out to the airplane. He stated that during his re-inspection and verification of the oil service; he discovered both engine oil caps incorrectly installed. He corrected both caps and signed off the logbook.
