FSDO INSPECTOR DISCOVERED ACFT AD NOT COMPLETED AS ACFT WAS PRESENTED FOR A PLT CHK RIDE AND CERTIFICATION FOR CHARTER OP.
Narrative
IN PROCESS OF PLACING AN SMT INTO CHARTER SVC ACFT LOGS ORIGINALLY INSPECTED BY DIRECTOR OF OPS (MYSELF) AND DIRECTOR OF MAINT WITH NO ITEMS FOUND. ACFT RECORDS AGAIN INSPECTED BY FAA FSDO PERSONNEL 9/TUE/91 WITH NO ITEMS FOUND. ON 10/WED/91 THE ACFT WITH MYSELF AND 2 OTHER PLTS WENT TO FAA FOR CHK RIDES; AT THIS TIME FAA MAINT PERSONNEL (THE SAME PERSON) FOUND AN AD OUTSTANDING. THE FOLLOWING DAY A FERRY PERMIT WAS OBTAINED TO THE REPAIR STATION WHICH PERFORMED THE LAST INSPECTION. IN OUR AFTER THE FACT RESEARCH ANOTHER AD WAS FOUND NOT PROPERLY SIGNED OFF. THE AD HAD IN FACT NOT BEEN COMPLETED WITH AND WAS TAKEN CARE OF WHILE WE WAITED. 91.417 (A)(2)(V) IS VERY SPECIFIC ABOUT HOW AD'S SHOULD BE RECORDED HOWEVER; AFTER 30 HRS OF RESEARCH PLUS REVIEW BY THE FAA ITEMS ARE STILL MISSED BECAUSE OF LACK OF STANDARDIZATION OF RECORDING PROCS! I HAVE DISCUSSED THIS WITH OUR MAINT INSPECTOR AND HE SMILES AND SAYS IT'S OUR PROBLEM. I FEEL STRONGLY THE FAA SHOULD MANDATE THE FORMAT AS THEY MANDATE THE AD'S ON ACFT SUCH AS A 20 YR OLD SMT WHERE AD RECORDING FORMAT HAS CHANGED 3 OR 4 TIMES IT IS IMPOSSIBLE TO ABSOLUTELY VERIFY ALL THAT'S BEEN DONE!
