Synopsis
B737 flight crew reported a Cockpit Fire Extinguisher fell from its mounts and struck the Emergency Gear Extend Door. On take off; all gear unsafe lights illuminated and the landing gear did not retract. The flight crew contacted maintenance and was advised to re-close the door and the landing gear were successfully retracted.
Narrative
On the ground just prior to push after flight deck pre-flight. The first officer got into his seat and the fire extinguisher fell off its mount. Onto flight deck floor. We put it back on mount but latch would not secure very tight; but held the extinguisher. During the after takeoff flow and checklist we found the gear lights disagreed and gear did not retract. 3 red and 3 green. Overhead lights all green. Ran QRH for gear handle in off position which determined gear down and locked. At the time we were 20;000 lbs. overweight. Asked for holding to burn fuel below max landing weight approx. 2.0 hours. Passenger and Flight Attendant briefed and checks completed. During communication with Dispatch was told to check manual gear extension door that was not in our QRH FLOW FOR GEAR IN OFF POSITION. Door was unlatched due to fire extinguisher falling on it. We did not see it or catch it due to IMC terrain and high work load. We then got gear to operate normally after latching door closed. At that time we had burned too much fuel to continue to ZZZ1. Burned fuel down landed uneventfully under max weight. Fire extinguisher latch and manual gear ext door was written up and put in logbook.
Second reporter narrative
Prior to push back; the First Officer's chair was moved forward and fire extinguisher fell on ground due to loose latch. Fire extinguisher was reinstalled to mount. After non-eventful taxi; First Officer took off in IMC in an area of high terrain and at night. It was discovered during the after takeoff checklist that the gear failed to retract (all lights illuminated). We [requested priority handling] and went to holding to run checklists and trouble shoot the issue. After running the checklist and contacting Dispatch we were advised to check the manual gear door. The manual gear door was not latched and suspected to have been unlatched by the fire extinguisher falling on it prior to takeoff. After the manual gear door was closed the gear was able to be retracted. Due to fuel requirements we were no longer able to proceed to ZZZ1 and burned off fuel to land back at ZZZ below max landing weight. Uneventful landing and taxi back to gate.
