Synopsis

Air carrier ramp loading lead reported being notified that the aircraft cargo was improperly loaded and two cargo carts had not been loaded onto the aircraft.

Narrative

I was given an entire assist crew with a crew chief. Before loading the aircraft I was printing a NOTOC for dangerous goods; class 7 and 9. when I arrived to the gate I showed the assist crew chief the trip sheet with all the cargo carts information. I told him all cargo must be loaded in F2 with 20 bags and that F1 would have 44 bags. The entire crew acknowledged the information I provided them. I proceeded to go to the back of the aircraft and load the aft compartment. As I was loading the aft compartment; I noticed the assist crew chief was loading luggage. I approached him and asked if all cargo was loaded which he replied; Yes". Once he confirmed that all cargo was loaded I went into the system and placed the cargo in F2 compartment. I then proceeded to go to the cockpit and inform the Captain and the First Officer (FO) they had dangerous good class 7 and 9. When the flight left and I was on my way home I received a phone call from a Manager informing me that 2 cart were not loaded on the aircraft and the flight had misinformed cargo. I offered to go back but the manager informed me that he would figure it out and give me a call back. I realized the event occurred due to a phone call from the manager in charge that night The event occurred due to the assist crew chief not informing me that 2 carts were not loaded on the aircraft. If I was informed that the cargo was not loaded; I would have never placed the cargo inside the system. After the event occurred it is now a pending investigation. I would recommend that if a crew chief is assisting another crew chief; they should pay attention of what needs to go on the aircraft and inform the primary crew chief the correct information. When issue's like this happen not only can you put someone in a dangerous position but you expose passengers to a high risk due to weight and balance issues or worse it could possibly be the incorrect cargo information all because we as employees are not paying attention to instructions."
