ACR DEPARTS AFTER L ANGLE OF ATTACK VANE REMOVED DUE TO DAMAGE. LOG BOOKS SIGNED. DISPATCH SENDS AUTHORIZATION TO FLY.
Narrative
UPON BOARDING THE ACFT AT ONTARIO; THE GATE AGENT NOTICED THAT THE JETWAY APPEARED TO BE MIS-PARKED. THE EXTERIOR PREFLT WAS ACCOMPLISHED IMMEDIATELY; AND THE L ANGLE OF ATTACK VANE WAS FOUND TO BE DAMAGED. AT THIS TIME; CALLS WERE MADE TO OUR DISPATCHER; OUR MAINT COORD; AND CONTRACT MAINT PERSONNEL. BECAUSE THE AOA VANE WAS VERY LOOSELY ATTACHED; IT WAS REMOVED BY MAINT AS A FOD PRECAUTION. OUR MAINT COORD THEN SENT US A TELETYPE DISPATCH AUTHORIZATION TO DEFER THE L STALL WARNING SYS. THIS WAS ENTERED IN THE LOG BOOK AND A SUCCESSFUL TEST OF THE R STALL WARNING SYS WAS ACCOMPLISHED. (THIS MESSAGE CONTAINED NO REF TO CIRCUIT BREAKERS.) A HAND WRITTEN PLACARD WAS INSTALLED ON THE AFFECTED SYS; AND THE FLT WAS PUSHED BACK WITH MIN DELAY. ON TKOF FROM ONTARIO; THE L STICK SHAKER ACTIVATED AT LIFTOFF; AND REMAINED ON UNTIL LNDG. THE DECISION WAS MADE TO RETURN TO ONTARIO; AND A NORMAL APCH AND LNDG WAS ACCOMPLISHED. UPON ARR AT THE GATE; FLT CTL AND THE MAINT COORD WERE AGAIN NOTIFIED. THEY SENT A SECOND MESSAGE TO DEFER THE L STALL WARNING SYS; WHICH INCLUDED PULLING THE APPROPRIATE CIRCUIT BREAKERS. THE CREW FEELS THAT WE FOLLOWED THE INST OF BOTH MAINT MESSAGES TO THE LETTER. A SECOND TKOF WAS MADE; AND NO STALL WARNING INDICATIONS OCCURRED. THE FLT THEN CONTINUED UNEVENTFULLY TO THE DEST ARPT.
