B767 First Officers reported an engine failure when throttles were advanced to takeoff thrust. The Captain rejected the takeoff and called for Crash Fire Rescue to address the ATC reported fireball Tower observed before being towed back to the gate.

Synopsis

B767 First Officers reported an engine failure when throttles were advanced to takeoff thrust. The Captain rejected the takeoff and called for Crash Fire Rescue to address the ATC reported fireball Tower observed before being towed back to the gate.

Narrative

After an uneventful pre-flight and boarding; we pushed back on time and taxied to Runway XC at [Taxiway] 1. On takeoff; the captain advanced the throttles; commanded autothrottle;" and both engines spooled up in unison. Just as engines reached takeoff thrust there was a loud bang followed by significant yaw. The captain quickly rejected the takeoff and came to a stop on centerline; only having travelled several hundred feet from [Taxiway] 1. It was a very smooth low speed reject; RTO (rejected takeoff) did not engage; and brake temperatures never exceeded 1. As briefed by the Captain prior to push-back; the relief FO (First Officer) made the passenger announcement to remain seated and I notified Tower. The Tower reported a large fireball had been visible and (CFR) Crash Fire Rescue was requested. There were no indications of fire in the flight deck and the REJECTED TAKEOFF checklist was completed. We noticed that the R engine was displaying blank for N1 and oil pressure was in red; so we completed the ENGINE FIRE (R) or ENGINE SEVERE DAMAGE or SEPERATION checklist down to pulling the fire handle (without discharging) and the remainder of the checklist was not applicable since we were on the ground. CFR arrived and sprayed the engine with foam due to residual smoke. The FAs (flight attendants) reported that smoke had been visible from the cabin but the passengers were calm and had remained seated.After coordination with company; it was determined that we would be towed back to the gate for passenger egress. While that coordination was occurring; CFR made another pass with water into the engine to wash the foam.The company aircraft move team arrived; we completed the tow in supplementary procedure; and the aircraft was uneventfully towed back to gate XXX."

Second reporter narrative

After a non-eventful preflight and taxi out we were cleared for takeoff on Runway XC. The Captain brought the engines up and called for auto throttles to be engaged. The throttles advanced and the Captain said check thrust" and as soon as we reached takeoff power we heard a loud bang and the aircraft swerved to the right. The Captain called reject; brought the plane back on centerline and stopped. We ran the rejected takeoff checklist followed by the engine failure/severe damage checklist. After securing the engine the CFR (Crash Fire Rescue) surrounded the aircraft and sprayed the right engine. We talked to the flight attendants and they said they saw the engine smoking; but there was no smoke in the cabin. After the CFR did their checks; company Maintenance then looked at the plane and determined the plane would be safe to be towed back to the gate. We were towed back and the passengers exited via the jetway."
