Synopsis

PLT OWNER COULD NOT CTL ACFT ENG OUTPUT WHEN LEVELING OFF AFTER CLB OUT AND WAS FORCED TO LAND AT A NEARBY ARPT. THIS WAS THE FIRST FLT AFTER MAJOR ENG MAINT.

Narrative

INSTALLATION OF ENG PART WAS PERFORMED ON SMA ACFT. INSTALLATION WAS COMPLETED ON MAR/XX/92 AND FINAL ADJUSTMENTS ON MAR/XY/92. ACFT WAS STARTED AND RUN AGAIN WITH NO PROBLEMS. OWNER/PLT DEPARTED JGG AND AFTER REACHING CRUISE ALT COULD NOT RETARD ENG SPD WITH THROTTLE. MOVEMENT OF THROTTLE HAD NO EFFECT. PLT DIVERTED TO PHF AND DECLARED AN 'IFE.' ACFT LANDED ENG OFF WITHOUT DAMAGE OR INJURY. ACFT WAS TOWED TO A MAINT HANGAR ON PHF. INSPECTION REVEALED THAT THE CARB ARM/THROTTLE ROD END BOLT WAS MISSING. A TEMPORARY BOLT WAS INSTALLED SO PLT COULD RETURN TO JGG. AFTER LNDG AT JGG ACFT WAS PUT INTO MAINT HANGAR THAT INITIALLY PERFORMED REMOVAL AND INSTALLATION OF CARB AND HARDWARE WAS INSTALLED THAT IS CALLED FOR BY ACFT MANUFACTURER; SVC MANUAL; SVC BULLETINS; AND AIRWORTHINESS DIRECTIVES. IT IS MY OPINION THAT THE BOLT BROKE OR SHEARED ON TKOF RUN OR DURING CLBOUT AND VIBRATED FREE AND WAS LOST INFLT. FUTURE RECOMMENDATIONS: CHANGE ALL HARDWARE ON FUEL CTL DEVICES WHEN THEY ARE CHANGED TO NEW.
