Synopsis

STICK SHAKER AND AURAL TKOF WARNING HORN ACTIVATED ON TKOF. ACFT MAINT DISCREPANCY PROBLEM.

Narrative

THE FOLLOWING IS A SUMMARY OF THE EVENTS RELATED TO FLT OF MARCH 1992: CLRED FOR TKOF RWY 28R PIT. AFTER TKOF CLRNC: TURN R HDG 360; MAINTAIN 5;000 FT. JUST PAST V1 AT ROTATION; BOTH STALL RECOGNITION SYS SOUNDED WITH STICK SHAKERS; STALL LIGHTS AND BOTH HORNS. WARNING WAS DETERMINED TO BE FALSE AS PROPER AIRSPD PROFILE WAS MAINTAINED DURING ALL PHASES OF FLT. STALL TEST SWITCH WAS HELD IN BOTH SYS 1 AND SYS 2. THE WARNING WITH STICK SHAKERS; STALL LIGHTS AND HORNS CONTINUED. THE RIGHT STALL WARNING COMPUTER CIRCUIT BREAKER WAS PULLED; THE STALL WARNING STOPPED AND ALL SYS RETURNED TO NORMAL AFTER THE CIRCUIT BREAKER WAS RESET. THE PROBLEM WAS DISCUSSED WITH THE DISPATCHER AND MAINT CTL IN FLT AND WE AGREED TO PROCEED TO BTV. DUE TO FULL STALL WARNING SOUNDING; THE TWR'S TKOF INSTRUCTIONS STATED IN ITEM 1 ABOVE WERE NOT FOLLOWED. CLB WAS CONTINUED STRAIGHT AHEAD AND NORMAL NOISE ABATEMENT CLB WAS NOT FOLLOWED. PWR WAS LEFT AT TKOF PWR UNTIL STALL WARNING WAS DETERMINED FALSE. TKOF PWR DID NOT EXCEED FOUR MINS. DURING THE TIME THE STALL WARNING WAS SOUNDING; THE NOSE LEVEL WAS SO HIGH; COMS BTWN PLTS WAS EXTREMELY DIFFICULT. THE NOSE LEVEL WAS SO DISRUPTIVE; RADIO CONTACT WAS LOST WITH THE TWR FOR THE CHANGE OVER TO DEP CTL. THE CO-PLT INITIALLY XMITTED IN THE BLIND TO DEP; STATING THE NATURE OF THE PROBLEM. VOLUMES WERE TURNED TO THE MAX AND WE WERE THEN ABLE TO GET VECTOR HDGS AND AN ALT UNTIL THE ABNORMAL WAS RESOLVED AND DISPATCH CONTACTED. LATER THAN EVENING DURING A TELEPHONE CONVERSATION WITH THE PIT DEP CTLR; HIS COMMENTS AS FOLLOWS: OUR INITIAL CONTACT SCARED HIM DUE TO THE 'GOSH AWFUL NOISE' IN THE BACKGND. HOWEVER; ONCE THE CTLR UNDERSTOOD THE NATURE OF THE PROBLEM; ALL QUESTIONS WERE ANSWERED FOR HIM AND HE FULLY CONCURRED WITH OUR ACTIONS.
