Synopsis

Air carrier maintenance personnel reported HAZMAT had been shipped undeclared and without the proper documentation.

Narrative

ZZZ shipped loaned tool; HAZMAT UN 3363; 1956; 3481; undeclared and without a Dangerous Goods (DG) form to ZZZ1 via the daily courier on Day 0. At approximately XA:00 Day 0; Maintenance was let into the parts room with the tool at which time the Technician rolled the tool case to the courier staging area and stated it needed to go back to ZZZ1 that night. Staff on shift did not follow up to ensure there was a DG form created to move the tool and just loaded the tool with the rest of the items when the daily courier arrived; therefore leading to the undeclared shipment of HAZMAT.Suggestions: Retrain of all staff in materials that nothing moves without a DG form; especially when a part or tool is handed off by Maintenance with no paperwork; those situations should always be questioned. Retrain on the process of creating a DG form when needed and proper contacts and procedures if unsure on proper steps. Additional instructions and training for staff on how to handle movement of loaned tools.
