B777 First Officer reported the DG summary that had information on the loading location of the items was initially not provided. After the reporter contacted several people; Ramp finally provided a DG notification form that had all the required information that allowed the flight crew to depart.
Narrative
Upon ACARS initialization; the planned Dangerous Goods (DG) form automatically printed. This form included detailed item information for approximately 20 items of DG; but did not include the DG summary at the top of the form. The summary includes useful and required information; including the loading location of each item. There were also approximately 14 items which were missing from the form; presumably because they were being held off. This could not be verified without the DG summary information.I contacted Operations; Cargo Support; and the Chief Pilot. I also sent a DG acknowledgement code to reprint the form. The form reprinted without the summary information. We then received the final DG form; again without the summary. The form then reprinted several more times; possibly because of the people I contacted pushing the form to the airplane. In total; we received about 5 - 6 pilot DG notification forms; none of which included the summary information; including loading location. Without this information; we would not be in compliance accepting any of the DG items.At this point; someone from Ramp came to the cockpit carrying a dot matrix printout of the DG notification form. This form included the summary information; including all loading locations and confirmation that approximately 14 items had been held off. We needed to have the Ramp Lead come back to the cockpit to sign the form indicating all items had been properly loaded; secured; and were not leaking. Once this was accomplished; we departed.
