Synopsis

ACR IA SIGNS OFF BRAKE REPAIR WITHOUT PROPER CHK OF WORK. USES WRONG PARAGRAPH NUMBER FROM MAINT MANUAL.

Narrative

AS A LEAD IA; ASSIGNED A MECH TO BLEED BRAKES ON A LGT WHICH CAME IN WITH A LOG GRIPE OF DRAGGING BRAKES. BEING WE WERE SHORT-HANDED; I ASSISTED HIM BY APPLYING BRAKES WHILE HE BLED THEM. WHEN IT CAME TIME TO SIGN OFF THE WRITE-UP; THE MECH ASSURED ME HE FOLLOWED THE MAINT MANUAL; SO I OFFERED TO INPUT THE WRITE-UP INTO COMPUTER SO HE COULD START ANOTHER JOB. BEING I HELPED THE MECH BY APPLYING THE BRAKES; I ALSO SIGNED WRITE-UP WITH OTHER MECH. I INADVERTENTLY INPUT THE WRONG PARAGRAPH NUMBER OF THE MAINT MANUAL. THE ACFT WAS DISPATCHED; THE ENGS WERE STARTED; THE TOW BAR WAS DISCONNECTED; AND THE ACFT MOVED FORWARD; AND STRUCK THE TWO TRACTOR CAUSING DAMAGE TO THE ACFT. THE INVESTIGATION FOUND THAT THE LOCKOUTS WERE NOT RESET AFTER BLEEDING WHICH MEANS THAT THE MAINT PROC WAS NOT FOLLOWED. THE ERROR ON MY PART WAS I INPUT THE WRONG PARAGRAPH NUMBER AND TRUSTED THE MECH FOR FOLLOWING THE PROPER PROC. NEXT TIME; I'LL BE SURE TO RECHK MY TYPING AND FOLLOW UP ON WORK THAT I'M NOT FULLY INVOLVED WITH.
