Push-back personnel reported that the aircraft rolled forward and struck the tow bar after the Captain was advised to set brakes during pushback. Push crew reported that the Captain replied that he thought he had set the brakes when instructed to do so.
Narrative
During pushback; I pushed the aircraft tail north. After I completed the pushback I called to the flight deck to set brakes. The pilot then set flight deck to ground; brakes are set; I then gave the signal to my wingwalker to disconnect the towbar and the headphones from the aircraft. At that point my wingwalker disconnected the headphones first and then disconnected the towbar. Once the towbar was disconnected and it dropped down the aircraft immediately started to roll forward. I started to back up in the pushback to avoid the pushback from getting crushed. After finally getting the pilots attention and he re engaged his brakes; I had my wingwalker reconnect the headphones and get back in contact with the pilot. I asked him if he had set brakes and to that he replied 'Yea I set the brakes; or at least I thought I did.' I saw the aircraft moving forward after the towbar was unhooked. Cause: I asked the pilot if he had set brakes and to that he replied 'Yea I set the brakes; or at least I thought I did.'
Second reporter narrative
i was wing walking the captain side. We came to the drop off point i was given the cross/stop signal. i gaved the cross wand signal to pilots and the aircraft started moving forward. i tried to call the attention of pilots no one acknowledged after a few attempts pilot set the aircrafts brakes and stopped. The aircraft began to move forward after tow bar disconnected.Cause: the accident occurred basically from miscommunication between pilot and push driver.
