Narrative
PRACTICE ILS RWY 4 APCHS UNDER VFR CONDITIONS AT FAY PUT APPROX XA00. I WAS NOT ON AN IFR FLT PLAN. I WAS FLYING THE APCHS UNDER THE HOOD WITH A SAFETY PLT PRESENT. FAY'S ARSA WAS OTS. WITHOUT RADAR VECTORS TO INTERCEPT THE LOC; I EXECUTED THE FULL ILS APCH INCLUDING THE PROC TURN. 2 ILS APCHS WERE MADE TO RWY 4; BOTH WITH THE OPTION. INSTRUCTIONS FROM THE FAY TWR INCLUDED RPTING DOONE (LOM) INBOUND. DURING THE FIRST APCH; RWY 4 WAS THE ACTIVE RWY. DURING THE SECOND APCH; THE ACTIVE RWY CHANGED TO RWY 22 AND THE TWR INFORMED ME I MAY BE REQUIRED TO COMPLETE THE ILS RWY 4 APCH WITH A LNDG ON RWY 22. DURING THIS SAME APCH; I DID NOT INFORM THE FAY CTL TWR WHEN I WAS DOONE INBOUND. AT THE DECISION HT FOR RWY 4; I REMOVED THE HOOD AND DISCOVERED THAT ANOTHER ACFT THAT HAD BEEN INBOUND TO RWY 22 HAD LANDED AND WAS AT THE MIDPOINT ON THE RWY. I EXECUTED THE MISSED APCH AT THE SAME TIME THE TWR ADMONISHED ME FOR NOT RPTING DOONE INBOUND. THIS SIT WAS NEITHER THREATENING TO MY ACFT NOR THE ONE ON THE RWY. HOWEVER; THE SIT DID CAUSE EXCITEMENT IN THE CTL TWR. THE CONTRIBUTING FACTORS WERE FAILURE TO PROVIDE A POS RPT WHEN DOONE INBOUND AND MY SAFETY PLT NOT INFORMING ME TO BREAK OFF THE APCH DUE TO TFC ON THE RWY. SUPPLEMENTAL INFO FROM ACN 241987: ATC RADAR OTS. RWY CHANGING FROM 4 TO 22. PLT'S FAILURE TO CALL THE LOM INBOUND. SAFETY PLT DID NOT TAKE CHARGE WHEN RPT NOT MADE OR LATER WHEN AIRLINER LANDED ON RWY AND PLT CONTINUED APCH. SAFETY PLT RELUCTANT TO INTERRUPT PLT. TWR CTLR'S ATTN DIVERTED BY LNDG ACFT AND BUILDING FIRE IN PROGRESS 5 MI NW OF ARPT. I HAVE AVOIDED GIVING THE IMPRESSION OF TRYING TO ASSUME THE ROLE OF ACFT COMMANDER. IN 17 YRS OF FLYING AND TRYING TO BECOME AN AVIATOR I HAVE NOT SEEN ANY MATERIAL ON THE DUTIES OF A SAFETY PLT. IN THIS CASE; I ALLOWED MY DUTIES TO CONDENSE TO THE MINIMUM AND DID NOT SUPERVISE ENOUGH.
