Synopsis

AN ACI FOUND SEVERAL SCREWS MISSING ON AN ACR LTT FAIRING.

Narrative

ACFT WAS PICKED UP IN BOS FOR A SCHEDULED FLT TO PHL THEN RIC. I DESIGNATED THE COPLT TO DO THE EXTERIOR INSPECTION. THE COPLT RPTED BACK TO ME THAT THE ACFT WAS AIRWORTHY FOR FLT. I THEN DID MY WALK AROUND THE ACFT AND FOUND IT ALSO AIRWORTHY FOR FLT. FLT LEFT BOS FOR PHL AND THEN RIC WHERE THE CREW OVERNIGHTED. ON NOV/TUE/93; A PREFLT AND POST-FLT INSPECTION OF THE ACFT WAS PERFORMED FOR EVERY LEG FROM RIC-PHL-BOS-PWM. UPON LNDG IN PWM; THE CREW WAS MET BY AN FAA SAFETY INSPECTOR WHO; UPON COMPLETION OF HIS POST-FLT INSPECTION; FOUND A NUMBER OF SCREWS MISSING FROM THE FAIRING ON THE UNDERSIDE OF THE ACFT. ACFT WAS DOWNED UNAIRWORTHY; A MECH WAS CALLED TO REPLACE THE SCREWS AND WAS PROPERLY SIGNED OFF AIRWORTHY IN THE MAINT LOGBOOK OF THE ACFT. CALLBACK CONVERSATION WITH REPORTER REVEALED THE FOLLOWING: THE RPTING CAPT STATES THAT HE HAS RECEIVED A LETTER OF INVESTIGATION FROM THE FAA REGARDING THE MISSING SCREWS; TO WHICH HE REPLIED. HIS ACR HAS GIVEN DISCIPLINARY ACTION TO THE FLCS AND MECHS INVOLVED IN THIS INCIDENT. THE FLC RECEIVED 3 DAYS OFF WITHOUT PAY. HE DOES NOT KNOW THE PENALTIES GIVEN THE MECHS. HE BELIEVES THAT THERE WERE SEVERAL FLCS SENT LETTERS; POSSIBLY AS MANY AS 6.
