WHILE TAXIING ON 1 ENG; AN ACR LGT HIT A TERMINAL BUILDING.

Narrative

I WAS TOLD BY MY SUPVR TO TAXI ACR LGT TO W PAD ON #1 ENG. WHILE I WAS MARSHALLED IN; I FELT I WAS BEING MARSHALLED TOO CLOSE TO THE BUILDING; SO I APPLIED BRAKE. NO HELP. PULLED PARKING BRAKE. NO HELP. SHUT ENG AND ROLLED INTO BUILDING. SUPPLEMENTAL INFO FROM ACN 258783: WHEN I HEARD L SEATER SAY WE HAVE NO BRAKES; WE BOTH PUMPED THE BRAKES. SUPPLEMENTAL INFO FROM ACN 258897: MR A FROM NTSB AND MR B FROM FAA SHOWED UP TO INSPECT THE ACFT. THE ACFT WAS GOING TO BE TAXIED TO W PAD. SINCE THERE WAS DAMAGE TO THE #2 ENG THRUST REVERSER; WE TOLD MECH O TO TAXI THE ACFT WITH 1 ENG; TO PREVENT FURTHER DAMAGE TO THE ENG. SUPPLEMENTAL INFO FROM ACN 258778: THE INSPECTORS ASKED US NOT TO PERFORM ANY CORRECTIVE ACTION ON THE THRUST REVERSER OR ANY OTHER MAINT ON THE ACFT UNTIL THEY ARE DONE WITH THEIR INSPECTION. ACFT HAD HIT THE N TERMINAL DURING THE TAXI MOVE. WHEN I ARRIVED AT THE ACFT; THE L SEAT DRIVER HAD TOLD ME HE LOST ALL BRAKES WHILE APCHING THE TERMINAL. THERE WERE NO INJURIES INCURRED IN THIS MISHAP. THE ACFT IS DAMAGED ON THE FORWARD L SECTION AREA AND THE CAUSE OF THE BRAKE FAILURE IS UNDER INVESTIGATION. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTING MAINT MGR STATES THAT THE REVERSER COMING OFF WAS PROBABLY CAUSED BY A FAILURE OF A JACKSCREW IN THE REVERSER MECHANISM. THIS HAPPENS TO THIS CARRIER ABOUT TWICE A YR; BUT THE REVERSER SELDOM FALLS OFF. THE FAA AND NTSB ARE CONCERNED BECAUSE OF THE FREQ OF THE OCCURRENCES. THE FAA AND NTSB ARE PLANNING NO ACTION AGAINST THE ACR. THE PROB OF THE ACFT RUNNING INTO THE TERMINAL WAS CAUSED BY AN ERROR BY THE MECHS -- THEY DID NOT HAVE THE ACFT PROPERLY CONFIGURED TO TAXI ON 1 ENG. THE ANTISKID SWITCH WAS IN THE WRONG POS AND THE ELECTRICAL BACKUP FOR THE 'YELLOW' HYD SYS WAS NOT ON. THE RPTR BELIEVES THAT DAMAGE TO THE LGT WAS CONFINED TO A CUT TIRE. THE MECHS THAT TAXIED THE ACFT HAD HAD DISCIPLINARY ACTION TAKEN BY THE COMPANY.
