Synopsis

ACR MLG ACFT DEICED IN LIGHT SNOW SHOWERS BUT LEAVES WITHOUT HAVING COMPLETE DEICE RPT FROM GND CREW.

Narrative

FARS AND COMPANY POLICY REQUIRE THAT THE PERSON RESPONSIBLE FOR DEICING AN ACFT RPT THE FOLLOWING INFO TO THE PLT(S): ACFT TAIL NUMBER TYPE DEICING FLUID APPLIED; PERCENT MIXTURE OF FLUID(S); TIME OF INITIATION OF FINAL PHASE OF DEICING; AND A STATEMENT THAT THE ACFT CRITICAL AREAS ARE FREE OF CONTAMINATION AND THE POST-APPLICATION CHK IS COMPLETE. NO SNOW OR CONTAMINATION WAS ON THE ACFT PRIOR TO DEICING. BUT SINCE THERE WERE LIGHT SNOW SHOWERS; WE DECIDED TO DEICE JUST TO BE EXTRA SAFE. THE POST APPLICATION RPT WE RECEIVED (OVER COMPANY RADIO) WAS INCOMPLETE; AND ALTHOUGH I REMINDED HIM THAT IT WAS REQUIRED; THE CAPT DID NOT INSIST ON RECEIVING A PROPER RPT PRIOR TO TAXIING. SINCE I HAD JUST CHKED THE ACFT FROM THE OUTSIDE PRIOR TO PUSHBACK; AND DETERMINED THAT DEICING WAS NOT REQUIRED IN THE FIRST PLACE; I DIDN'T PRESS THE ISSUE. HOWEVER; HAD DEICING BEEN NECESSARY; I WOULD HAVE EXERCISED COCKPIT RESOURCE MGMNT SKILLS AND INSISTED ON A PROPER; COMPLETE DEICING RPT PRIOR TO MOVING THE ACFT. PART OF THE INITIAL AND RECURRENT TRAINING OF GND DEICING CREWS SHOULD BE A REQUIREMENT FOR AN ORAL TEST THAT INCLUDES THE TASK OF VERBALLY DEMONSTRATING SEVERAL EXAMPLES OF COMPLETE DEICING RPTS. ANOTHER WAY TO SOLVE THE PROB WOULD BE TO NOT ALLOW PLTS TO TKOF WITHOUT A COMPLETE; WRITTEN RPT. PLTS NEED TO BE TRAINED THAT THEY CANNOT GO WITHOUT A CORRECT; COMPLETE RPT.
