INSTRUCTOR SENT SPI ON X-COUNTRY WITHOUT PROPER ENDORSEMENTS. RAMP CHKED.

Narrative

I HAD MADE THE PROPER ENDORSEMENT IN THE STUDENTS LOG BOOK FOR A SOLO XCOUNTRY FROM NY10-DKK-ELM AND RETURN. AT DKK THE STUDENT WAS RAMP CHKED BY AN FAA OPS INSPECTOR AND HE FOUND THE FOLLOWING DISCREPANCIES: 1) THE STUDENTS/STUDENT LICENSE WAS NOT SIGNED OFF FOR SOLO XCOUNTRY. 2) THE STUDENTS LOG DID NOT HAVE A CURRENT 90- DAY SOLO ENDORSEMENT IN IT. THE STUDENT HAD BEEN PRIMARILY FLYING WITH MY OTHER INSTRUCTOR AND I HAD ASSUMED THAT HE HAD BEEN SIGNED OFF AFTER 2 QUAL X-C FLTS. BEING THAT I AM THE OWNER AND CHIEF FLT INSTRUCTOR; I FEEL THAT I AM THE ONE WHO IS RESPONSIBLE FOR THE LACK OF ENDORSEMENTS. IT WAS AN INADVERTENT OVERSIGHT ON MY PART CAUSED PRIMARILY TO A LACK OF COM BTWN MYSELF AND MY OTHER FLT INSTRUCTOR. THE STUDENT HAD SATISFACTORILY COMPLETED HIS XCOUNTRY CHKOUT AND THERE WAS NEVER A 91- DAY PERIOD WHEN HE HADN'T RECEIVED FLT INSTRUCTION. SINCE THIS INCIDENT; I HAVE INSTITUTED A HIGHLY VISIBLE STATUS BOARD WITH EACH STUDENT'S NAME; INITIAL SOLO ENDORSEMENTS; INITIAL XCOUNTRY STUDENT ENDORSEMENT; LAST 90-DAY CHK AND WHEN DUE. A CHKOFF LIST IS ALSO IN THE STUDENT'S FOLDER; WHICH IS KEPT AT THE SCHOOL. WE ARE ALSO GOING TO HAVE A WKLY MEETING TO DISCUSS THE STATUS OF EACH OF OUR ACTIVE STUDENTS.
