RPTR MECH PERFORMING 'A' CHK HAS ACFT REVERSER DISCREPANCIES REQUIRING ACFT RETURN TO MAINT.

Narrative

ACR X; COLUMBUS; OH. ACFT IN FOR A CHK INSPECTION. ON NOV/MON/94 ACFT XXXX I WAS ASSIGNED THE ENG PART OF THE A-CHK. I PERFORMED AS REQUIRED BY THE WORK CARD WITH REGARDS TO THE LUBRICATION OF THE THRUST REVERSER LINKS. IN THE ABSENCE OF ANTI-SEIZE; I USED A COMPOUND KNOWN AS XXXX; AN ADEQUATE SUBSTITUTE ACCORDING TO A LEAD MECH AT ACR X. THE ANTI-SEIZE COMPOUND COMPANY STOCK NUMBER 00- 0340-3-0047 HAS NEVER BEEN AVAILABLE IN COLUMBUS TO USE AS CALLED OUT FOR IN THE WORK CARDS UNTIL NOV/XF/94. THE WARNING TAGS WHICH THE WORK CARDS CALL OUT FOR WHEN DEPLOYING THE REVERSERS; HAVE NEVER BEEN READILY AVAILABLE OR USED UNTIL AFTER NOV/XC/94. THE AIRPLANE CAME BACK TO THE GATE WHILE TAXIING. MECH FOUND R THRUST REVERSER IN THE BYPASS POS WITH THE BYPASS ACCESS DOOR CLOSED. THERE WERE AT LEAST; TO MY KNOWLEDGE; 3 PEOPLE IN THE AREA OF THE R ENG THE NIGHT BEFORE. CORRECTIVE SUGGESTION: WHEN PAPERWORK CALLS OUT FOR A NEW COMPOUND OR LUBRICANT IT SHOULD BE SENT OR A CHANGE NOT MADE UNTIL EVERY STATION HAS IT READY TO GO AND USE. CORRECTIVE SUGGESTION: MORE ATTN SHOULD BE PLACED ON SAFETY WITH REGARDS TO WARNING TAGS. CORRECTIVE SUGGESTION: THRUST REVERSER BYPASS LEVERS SHOULD BE REPLACED RATHER THAN BOLTED AND SAFETY PINNED TOGETHER WHEN A MEL ON A REVERSER IS CLRED. A REVERSER BYPASS DOOR CANNOT BE CLOSED WITH REVERSER IN THE BYPASS CONDITION THE WAY MCDONNELL-DOUGLAS DESIGNED IT.
