Narrative
AT ABOUT THE TIME OF THE 80 KT CALL ON THE TKOF ROLL THE FO STATED; 'I HAVE NO AIRSPD.' I TOOK MY EYES OFF THE RWY AND LOOKED INSIDE AT MY INDICATED AIRSPD. IT TOOK ME A COUPLE OF SECONDS TO ESTABLISH THAT I ALSO HAD NO AIRSPD AND I SAID; 'I DON'T EITHER.' THEN I LOOKED OUTSIDE TO SEE HOW MUCH RWY WAS LEFT; I GLANCED AT THE STANDBY AIRSPD AND SAW BOTH 100 KTS AND 120 KTS; AND I ELECTED TO CONTINUE AS THE SAFEST COURSE OF ACTION. UNFORTUNATELY; ONCE AIRBORNE; I HAD SEVERAL ABNORMAL WARNINGS HAPPEN AT ONCE AND I REALIZED THAT I HAD NO AIRSPD; ALTIMETER; VERT SPD; OR PITCH. I DECLARED AN EMER. HAVING DEPARTED RWY 28R AT PIT; TWR SAID; 'ROGER; FLT XX TURN R FOR R TFC FOR RWY 28R.' I LEVELED OFF AT 1500 FT AGL ON THE RADAR ALTIMETER. USING PRIMARILY VISUAL CUES; BUT USING THE STANDBY INSTS AS BACK-UP; I FLEW THE R HAND TFC PATTERN AND MADE A NORMAL VISUAL APCH TO RWY 28R FOLLOWED BY A NORMAL LNDG (ALTHOUGH OVERWT) USING NORMAL REVERSE AND NORMAL BRAKES. UPON MAINT INSPECTION IT WAS FOUND THAT; DURING A 'STRING CHK' THE NIGHT BEFORE; MAINT HAD NEGLECTED TO 'PLUG IN' THE AIR DATA COMPUTERS. SUPPLEMENTAL INFO FROM ACN 304105: IT ALSO TROUBLES ME THAT THERE IS NO COCKPIT INDICATION THAT THE PITOT-STATIC SYS IS HOOKED UP. ON THE SURFACE; WE THOUGHT WE HAD ONLY LOST AIRSPD; NOT BOTH AIR DATA COMPUTERS AND ASSOCIATED SYS. THERE SHOULD BE SOME LIGHT OR WARNING. MAINT SAID THIS IS THE 3RD TIME THIS HAS HAPPENED. HAD THE WX BEEN IFR; I WOULD HAVE BEEN ADAMANT ABOUT REJECTING THE TKOF. SUPPLEMENTAL INFO FROM ACN 303904:I INSTALLED THE RADIO RACK. SOMEONE THEN CALLED ME OVER WITH A QUESTION ABOUT THE ENG CHANGE. I THEN RETURNED TO INSTALL THE COMPUTERS. THERE WERE NO STREAMERS OR MARKINGS ON ANY LINES TAKEN OFF THE RADIO RACK OR COMPUTERS. WHEN I INSTALLED THE ADC I THOUGHT I WOULD ALSO CHK TO SEE IF THEY NEEDED A CHKOUT. I CHKED THE MAINT MANUAL AND THERE WERE NO CHKS REQUIRED THE ADC HAD QUICK DISCONNECTS. I WAS INTERRUPTED SEVERAL TIMES THAT NIGHT I MUST HAVE FAILED TO CONNECT THE LINES TO THE FRONT OF THE ADC BOXES. THE CONNECTIONS MUST HAVE BEEN TIED OUT OF SIGHT WITH NO STREAMERS. I REALIZE THIS WAS A SERIOUS MISTAKE. I DEEPLY REGRET THAT THIS SIT HAPPENED. SUPPLEMENTAL INFO FROM ACN 304312:I KNEW THERE WERE SOME AVIONICS REMOVED FROM THE 'E AND E' COMPARTMENT WHERE THE STRINGER REPAIR TOOK PLACE. WHEN I CLOSED OUT THE OM-7 AND THE LOGBOOK I KNEW COMPANY PROC WAS; FOR THE LINE DEPT TO RE-INSTALL ANYTHING THEY HAD REMOVED; SO THE SHEET METAL DEPT COULD ACCOMPLISH THE REPAIR. ANYTHING THAT THEY REMOVE AND THEN RE-INSTALL IS LOGGED ON A FORM 'ME-189.' SO I FELT CONFIDENT IN THE COMPANY PROC TO SIGN THE OM-7 AND LOGBOOK. AFTER MY OWN INVESTIGATION; I FOUND OUT THE OM-7 AND THE ME-189 FORMS WERE MISSING FROM OUR LOGS AND RECORDS DEPT. NO ACCOUNTABILITY FOR THE ADC REMOVAL AND RE-INSTALLATION HAS BEEN CLAIMED.
