Narrative
BATTLING A SUMMER COLD; ENTIRE CREW AND I ARRIVED 1 HR PRIOR TO DEP. ACFT AND RAMP COOLED TO 90 DEGS DURING EVENING -- VERY HOT. SET UP COCKPIT AFTER PREFLT FOR A PICAH 5 DEP PRIOR TO CAPT'S ARR WITH PAPER WORK. INITIATED CHKLIST AND AT 'FLT INSTS AND BUGS' HE CALLED OFF DEP RADIAL OFF PXR; DME TURN HDG 190 DEGS; CROSS VOR AT OR BELOW 3000 FT; 4 DME OUT; R TURN 190 DEGS INTERCEPT 160 DEG RADIAL TO TFD. GREAT -- JUST LIKE NORMAL DEP TO DFW. PROB NEITHER OF US NOTICED WAS FLT PLAN AND PDC READ ST. JOHN'S 1 DEP. TURN L 020 DEGS INSTEAD OF R 19 DEGS. DEP CAUGHT OUR TURN APPROX 45 DEGS INTO IT AND CORRECTED US BACK TO 020 DEGS. NO OTHER TFC NEAR BY; NO CONFLICTS. CAUSES: I HAVE FLOWN THIS TRIP SEVERAL TIMES IN RECENT PAST AND ALWAYS WENT TO TFD. MY EXPECTATION WAS TO GO TO TFD. I HAD SET IT UP AND CAPT READ BACK THE PICAH DEP TO TFD. IN MY MIND AND HIS WE WERE GOING TO TFD. CAPT'S FLT PLAN AND CLRNC SAID DIFFERENT. I HAD READ XPONDER CODE; DEP FREQ AND ALT FROM PDC BUT SOMEHOW MANAGED TO FAIL TO SEE NEW DEP ROUTING. CAPT HAD READ ROUTING BUT READ BACK WHAT I HAD SETUP. NEITHER OF US READ CLRNC SITTING IN FRONT OF US. WE DID IT JUST LIKE WE HAD IN RECENT PAST. SOLUTION: ESPECIALLY WITH PDC SINCE YOU DO NOT HEAR CLRNC IS TO READ AND VERIFY WITH EACH OTHER THE ROUTING TOGETHER. SUPPLEMENTAL INFO FROM ACN 309673: I HAD REVIEWED THE FLT PLAN IN OPS. DURING THE 'BEFORE STARTING ENGS' CHKLIST AT THE POINT OF REVIEWING THE NAV PANEL SETUP; WHICH THE COPLT HAD ALREADY DONE; I DID NOT VERIFY THE SETUP WITH THE PLAN OR THE CLRNC. MY MANUAL WAS OPENED TO THE PHX PICAH 5 DEP INSTEAD OF THE SJN 1 DEP AND THE NAV RADIO SETUP MATCHED THE PICAH 5. THE ERROR WAS CAUSED BY MY FAILURE TO XCHK THE FLT PLAN; CLRNC; AND NAV SETUP INDEPENDENTLY FROM THE COPLT. HE; TOO; HAD FAILED TO FOLLOW THAT PROC.
