Synopsis

THRUST REVERSER LOW PRESSURE ACCUMULATOR WARNING LIGHT ILLUMINATES DURING TKOF.

Narrative

DURING TKOF ROLL AT STL; I NOTICED THAT THE R THRUST REVERSER ACCUMULATOR LOW LIGHT HAD COME ON; ON THE OVER HEAD ANNUNCIATOR PANEL. OUR SPD WAS NEAR 100 KTS AND BECAUSE THRUST REVERSERS ARE DEFERRABLE; I DECIDED NOT TO ABORT THE TKOF. COMPANY POLICY PRECLUDES ABORTS FOR ANYTHING THAT DOES NOT IMMINENTLY AFFECT THE SAFETY OF FLT. ON LNDG AT LGA; THE R THRUST REVERSER DID NOT DEPLOY. THE LNDG WAS NORMAL WITH NO DIFFICULTY STOPPING. MAINT DISCOVERED THE R THRUST REVERSER HAD BEEN SHUT OFF EXTERNALLY. THIS WOULD NOT NORMALLY BE NOTICED DURING WALK AROUND INSPECTION BECAUSE THIS SHUTOFF IS LOCATED BEHIND A CLOSED ACCESS COVER AND IS A MAINT FUNCTION. I DID NOT NOTICE THE REVERSER ACCUMULATOR LOW LIGHT BEING ON DURING TAXI AT STL; SO I ASSUMED THAT IT HAD COME ON DURING TKOF; BUT IN REFLECTING; IT MAY BE THAT I HAD MISSED THE LIGHT ON THE ANNUNCIATOR PANEL BECAUSE OF SINGLE ENG TAXI WHICH CAUSES MANY LIGHTS TO BE ILLUMINATED DURING TAXI. THE MD83 HAS A SCROLL TYPE ANNUNCIATOR PANEL; AND LIGHTS MOVE AROUND CONTINUALLY. I MAY HAVE BECOME COMPLACENT EXPECTING TO SEE LIGHTS ON DURING TAXI. BECAUSE THESE LIGHTS ARE ON THE OVERHEAD AND MY BIFOCALS BLUR ANYTHING IN THAT AREA UNLESS I MAKE A CONSCIOUS EFFORT TO TILT MY HEAD BACK. FROM NOW ON; I WILL MAKE A CONSCIOUS EFFORT TO READ EVERY LIGHT ON THIS PANEL BEFORE EACH TKOF. SUPPLEMENTAL INFO FROM ACN 316198: NEXT DAY WE FOUND OUT THAT THE ACFT HAD BEEN IN THE HANGAR FOR MAINT. THE MECHS HAD LEFT THE R REVERSER LOCKOUT PIN IN PLACE.
