Narrative
I WAS CAPT ON FLT PHX-MKE ON OCT/XX/95. WE WERE INBOUND ON AN ASSIGNED STAR. I HEARD ACFT IN FRONT OF ME GIVEN 30 MI W OF JANESVILLE RESTRS; SO I BUILT THAT POINT IN MY FMS LEGS PAGE IN ANTICIPATION. THE POINT TO MAKE THAT RESTR PASSED AND CTR CLRED ME TO CROSS 20 W OF JANESVILLE AT FL240. MY FO WAS OCCUPIED WITH ATIS AND COMPANY IN RANGE CALL. I ROGERED THE RESTR AND SET UP NEW POINT IN FMS. I RESET MCP (AUTOPLT) FOR FL240; BUT MISTAKENLY PLACED FL240 RESTR ON WRONG LINE IN FMS; I PLACED IT ON JANESVILLE INSTEAD OF 20 W. CTR GAVE ME A FREQ CHANGE AND ASKED IF I WAS GOING TO MAKE FL240. I CONFIRMED 20 W AND CHANGED FREQS. AT THIS POINT I REALIZED I WAS 3 MI FROM RESTR POINT AND HAD NOT STARTED DOWN. I DISCONNECTED AUTOPLT AND EXPEDITED DSCNT AS I CHKED IN ON NEW FREQ. I WAS THEN TOLD TO CROSS VEENA INTXN AT 10000 FT 250 KIAS. THERE WAS NO COMMENT FROM ATC ABOUT ALT OR TFC. MY MISTAKE WAS DUE TO A LACK OF SITUATIONAL AWARENESS AND TRUSTING THE AUTOMATED SYS I HAD PROGRAMMED INCORRECTLY. I HAD NOT INCLUDED THE FO IN THE CHANGE OF ALT PLAN AS HE WAS PREOCCUPIED AND THE TIME FRAME WAS SHORT. IF I HAD; HE COULD HAVE BACKED ME UP AND PREVENTED A POSSIBLE HAZARDOUS SIT FROM HAPPENING.
