LOGBOOK ERROR; OPEN ITEMS MISSED DURING FLC TECHNIQUE PREFLT PROC.
Narrative
I WAS THE FO ON FLT ARN TO ORD. I ARRIVED AT ACFT AND ACCOMPLISHED THE ORIGINATION PREFLT. WHEN I FINISHED MY WALKAROUND INSPECTION A MECH WAS IN THE JET BRIDGE FINISHING UP THE LOGBOOK. AT DEP I WAS NOT AWARE OF ANY OPEN ITEMS IN THE ACFT LOG. THE FLT PROCEEDED NORMALLY TO DEST. UPON ARR IN CHICAGO SEVERAL ITEMS WERE ENTERED INTO THE LOGBOOK AND MAINT WAS NOTIFIED OF THESE ITEMS AS REQUIRED. SUPPLEMENTAL INFO FROM ACN 319642: I WAS CAPT ON FLT. MAINT BROUGHT LOGBOOK TO ACFT SHORTLY BEFORE DEP. WE BRIEFLY DISCUSSED CORRECTIVE ACTION ON FUEL PUMP WRITE-UP AND THE DEFERRED CABIN VIDEO WRITE-UP. REVIEWED LOGBOOK PER PROCS. NO OPEN ITEMS NOTED. SUPPLEMENTAL INFO FROM ACN 319646: ON FLT AB ON OCT/SAT/95 I REVIEWED THE LOGBOOK. THE ACFT HAD JUST COME IN WITH 2 WRITE-UPS BOTH OF WHICH HAD BEEN TAKEN CARE OF BY ORD. I INITIALIZED THE BOOK AND LEFT. NEAR LAX A FLT ATTENDANT GAVE ME SOME WRITE-UPS. WHEN I WENT TO PUT THESE INTO THE NEXT PAGE OF THE BOOK FOLLOWING THE WRITE-UPS ON OCT/SAT/95; I FOUND 2 NON-AIRWORTHINESS ITEMS THAT HAD BEEN ENTERED IN THE NEXT BOOK ON OCT/SAT/95 NOT CLRED. FLT AC ON OCT/SAT/95 HAD 3 WRITE-UPS ON PAGE 3716737; PAGE NUMBER 3716738 WAS VOID BY MAINT; PAGE 3716738 HAD FLT XY; PAGE NUMBER 3716781 HAD 2 WRITE-UPS FROM FLT XZ ON OCT/FRI/95. I MISSED THE 2 ITEMS BECAUSE OF THEM BEING IN ANOTHER SET OF PAGES. SUPPLEMENTAL INFO FROM ACN 319647: DURING FLT ENRTE TO LAX; THE CABIN ATTENDANT DISCOVERED OPEN LOGBOOK ENTRIES FROM PREVIOUS FLTS TO AND FROM ORD-ARN-ORD THAT WERE ENTERED OUT OF SEQUENCE AND INTO THE NEXT PAD OF E-6 ENTRIES. THE ITEMS WERE ENTERED SUCH THAT NO INBOUND SIGNATURE FOLLOWED. ALSO; I DID NOT RECALL ANY OF THE OUT OF SEQUENCE ENTRIES APPEARING IN THE DECS KVA1 ENTRIES PRIOR TO FLT. THE ERRORS WERE RPTED TO LAX MAINT UPON ARR FOR LOGBOOK NUMBER CORRECTION; CORRECTIVE ACTION TO THOSE OPEN ENTRIES; AND RENUMBERING AND SEQUENCING OF THE E-6 LOGBOOK. I DID NOT CATCH THESE ENTRIES UPON REVIEW AT THE BEGINNING OF FLT; AS IT APPEARED THAT THE E-6 WAS PROPERLY SIGNED OFF AND ALL OPEN ITEMS WERE SIGNED OFF. FROM NOW ON; I WILL CHK THE E-6 FROM BACK TO FRONT FIRST; TO PRECLUDE RECURRENCE.
