LTSS OPERROR OCCURRED WHEN RPTR SWITCHED AN ACR TO ANOTHER CTR POS BELIEVING THE HDOF HAD BEEN ACCEPTED. RPTR ACCEPTED RESPONSIBILITY FOR LTSS.
Narrative
I WAS WORKING THE RADAR POS. AN ACR DEPARTED BUF FLT PLANNED TO JFK CLBING TO FL170. I ACCEPTED HDOF FROM ZNY. ACFT REQUESTED FL190 WHICH WAS APPROVED AND ENTERED INTO COMPUTER. ACFT REQUESTED ROUTING CHANGE OVER AVP LENDY 4 ARR. THIS WAS APPROVED BY ZNY AND ISSUED TO ACFT AND ENTERED INTO THE COMPUTER. ZBW NAS THEN FAILED AND WE WERE CHANGING TO DARC; THE BACKUP SYS. I TOLD MY 'RA' CTLR TO RELIEVE ME SO THAT I COULD DIRECT THE AREA TRANSITION. AFTER DEPRESSING THE CHKLIST I SAW THE HDOF ACCEPTED BY ZNY FOR ABOUT A SECOND. THEN WE TRANSITIONED TO DARC WHICH DID NOT SHOW THE HDOF COMPLETED. I ADVISED THE NEW CTLR DURING THE BRIEFING THAT THE ACR WAS RADAR WITH NEW YORK SECTOR 50; THE SAME SECTOR THAT HAD HDOF TO ME. AFTER OUR OVERLAP I DIRECTED THE AREA TRANSITION TO/FROM DARC/NAS. ABOUT 5 MINS LATER I WAS ADVISED THAT NEW YORK DID NOT IN FACT ACCEPT THE HDOF AND THEY WERE UPSET AS THEY HAD LTSS 800 FT AT 1 MI. I ACCEPT FULL RESPONSIBILITY AS ALL DATA DID NOT SUPPORT MY BELIEF THAT I SAW THE HDOF ACCEPTED. I THOUGHT AND EVEN BRIEFED THAT THE HDOF WAS ACCEPTED WHEN IN FACT; IT WAS NOT. THE FAILURE OF NAS AT A CRITICAL TIME; THE FAILURE ON MY PART TO VERBALLY CONFIRM THE HDOF COULD HAVE CONTRIBUTED TO THIS SCENARIO. IN THE FUTURE I WILL VERBALLY CONFIRM HDOFS DURING THE TRANSITION BTWN SYS. I AM MOST CONCERNED WITH MY SEEING SOMETHING HAPPEN THAT ALL RECORDS INDICATE DID NOT HAPPEN.
