Narrative

I FLEW CAPT PIC; PF; B747-2A; XX AIR; NRT TO ORD. INSTRUCTOR CAPT IN R SEAT. DEPARTED NRT ZZ05Z (ZZ05L); USED NRT SID-CHOSI 4 RWY 34. DEP CTL SAID; 'FLY HEADING 070 DEGS; INTERCEPT OTR-11 ON COURSE. (CVC 092 DEG RADIAL.) I HAD MORIYA VOR (SNE 114.0) TUNED FOR SID AND FAILED TO SWITCH OVER TO CVC VOR; SO I DIDN'T REALIZE I OVERSHOT OTR-11 AIRWAY. DEP SAID; 'YOU ARE 4 MI L OF COURSE; TURN S AND INTERCEPT TFC 20 MI OUT OF 21000 FT; DSND TO 11000 FT.' I WAS CLBING OUT OF 16000 FT. I TURNED TOWARD OTR-11 AND DSNDED; 336K AND 3000 FPM. ONE MIN LATER I WAS ON COURSE AND DSNDING THROUGH 13000 FT. DEP SAID; 'RESUME CLB TO FL330.' NO LOSS OF SEPARATION. NO TCASII WARNING. NO TFC DISPLAYED ON TCASII. LESSONS: I SHOULD HAVE AURALLY IDENTED ACTIVE VOR; OR ASKED OTHER CREW MEMBER TO IDENT MY VOR. INSTRUCTOR CAPT IN R SEAT WAS TAKING ON INS FIX CHK OVER CVC VOR; SO HE WAS OUT OF LOOP. NO HELP AT ALL. ALSO; THIS CHK REQUIRES ALL 3 INS IN WAYPOINT; SO I DIDN'T HAVE THE DIGITAL XTRACK. THIS CHK SHOULD BE DONE ABOUT 100 MI OFFSHORE; AFTER AWAY FROM BUSY; CROWDED ARPT AREA. PF AND PNF HAD A DEBATE OVER CVC ABOUT WHAT RADIAL WAS CORRECT FOR OTR-11 AIRWAY. DISTR AND DEBATE WHEN I SHOULD HAVE BEEN TURNING CVC VOR FOR OUTBOUND RADIAL. CAPTS DON'T MAKE REAL GOOD COPLTS. BEWARE.
