Narrative
ON TKOF THE CAPT FLEW THE SID AS PUBLISHED. HE INTERCEPTED THE CVG VOR 035 DEG RADIAL. DEP CTL CALLED AND ASKED OUR HDG -- I REPLIED 035 DEGS. THE CTLR ASKED WHO GAVE US THAT HDG. I REPLIED WE WERE FLYING THE SID AND WE HAD NOT BEEN ASSIGNED A HDG AND THAT THE TWR HAD ONLY ISSUED A TKOF CLRNC. THE CAPT TURNED BACK TO THE RWY HDG -- 360 DEGS AND I ASKED WHAT HDG HE WANTED. THE CTLR SAID TURN R TO 050 DEGS AND INTERCEPT THE 035 DEG RADIAL OF CVG. I TOOK A SECOND LOOK AT THE SID AND REALIZED WE HAD MISREAD THE INSTRUCTIONS. THE SID SAID TO FLY RWY HDG OR ASSIGNED HDG FOR VECTORS TO ENRTE. WE MISSED THE VECTOR PART AND FLEW THE SID AS PUBLISHED WHICH WAS THE CVG 5. I BELIEVE PART OF THE PROB IS THE CAPT DOES NOT FOLLOW COMPANY POLICY. THE BEFORE TKOF CHKLIST CALLS FOR A TKOF BRIEFING THAT BRIEFS INITIAL HDG(S) AND ALT CLRNC. THE CAPT RESPONDED TO THE CHKLIST AS HE HAS FOR THE PAST SEVERAL DAYS 'AS BRIEFED.' THIS IS CONTRARY TO COMPANY POLICY. HE HAD NEVER BRIEFED THAT HE INTENDED TO TURN ON THE 035 DEG RADIAL WITHOUT A VECTOR.
