Narrative
UNINTENTIONAL GEAR UP LNDG DURING FIRST TFC PATTERN AFTER AREA WORK UPGRADE PLT LANDED ACFT WITH LNDG GEAR UP. TRAINING FLT BEGAN WITH STUDENT COMPLETING CLOSED BOOK EXAM. INSTRUCTOR BRIEFED STUDENT ON GENERAL FLOW OF MISSION; VFR DEP; AREA WORK TO INCLUDE STEEP TURNS; STALL SERIES; SLOW FLT DEMONSTRATION; PARTIAL PANEL FLT; SIMULATED FORCED LNDG; AND OTHER MANEUVERS AS REQUIRED TO SATISFY CLUB RULES. AFTER COMPLETING AREA WORK; ACFT WAS FLOWN TO LCL MUNICIPAL ARPT FOR PRACTICE LNDGS. STUDENT LEFT LNDG GEAR DOWN WHILE ENRTE TO ARPT AND WHEN ACCOMPLISHING BEFORE LNDG CHK PLACED LNDG GEAR UP. DUE TO TFC IN PATTERN INSTRUCTOR PLT FAILED TO MONITOR ACFT CONFIGN AND DID NOT SEE UNSAFE/STOWED LNDG GEAR CONFIGN. ACFT WAS FLOWN AND LANDED GEAR UP; DAMAGING PROP AND UNDERSIDE OF ACFT. HUMAN FACTORS INCLUDE: 1) UNIQUE CONFIGN OF LNDG GEAR LEVER OR 'JOHNSON SWITCH' WHICH PLACES THE LNDG GEAR DOWN WHEN THE HANDLE IS IN THE UP POS. 2) THE GEAR WARNING HORN CANNOT BE HEARD WHEN USING A HEADSET. 3) THE CHKOUT FLT HAD GONE WELL AND THE INSTRUCTOR PLT MAY HAVE LET HIS GUARD DOWN. 4) UPGRADING PLT HAD NEVER FLOWN A LOW WING COMPLEX ACFT WITH A JOHNSON SWITCH. 5) UPGRADE PLT WAS IN PROCESS OF CHANGING JOBS; CHANGING LOCATION WITHIN THE UNITED STATES.
