Synopsis

A B737-400 IMPROPER OP OF ENG FUEL LEVER LEFT IN IDLE CUTOFF POS WITH BOOST PUMPS ON.

Narrative

ACFT IN AUG/XX/96 HAD AN ENG FAILURE AND CHANGE. ON THE EVENING OF AUG/THU/96 I WAS WORKING A 'B' CHK ON THE ACFT AT ZZZ. I WAS DOING AN IGNITION CHK AFTER THE ENGS WERE RUN FOR CHIP DETECTORS CHK. I SWITCHED THE APU AIR TO OFF AND AFTER #1 AND #2 ENG SPOOLED DOWN. I CALLED DOWN TO LET THE GUYS ON THE GND KNOW TO CHK FOR IGNITION. I TURNED THE #1 ENG START VALVE SWITCH TO GND AND POSITIONED THE FUEL CUTOFF TO IDLE. I CALLED DOWN AGAIN AND HEARD NO REPLY. I LEFT THE SWITCH AND FUEL IN POS AND DID THE SAME FOR #2 ENG. AFTER APPROX 10 SECONDS; ONE OF THE GUYS CALLED UP TO ME AND SAID THAT #2 ENG HAD A FLAME AND FOR ME TO MOTOR THE ENG. I SWITCHED APU AIR TO ON AND SAW IMMEDIATELY THAT THE FUEL CUTOFFS WERE STILL IN IDLE. I PUT THE LEVERS IN CUTOFF POS AND #1 START VALVE TO OFF. I MOTORED THE ENG FOR APPROX 30 SECONDS. THE MAN ON THE GND TOLD ME THAT THE FLAME LASTED A FEW SECONDS AND WENT OUT AS SOON AS THE ENG STARTED MOTORING. I HAD NO INDICATION OF EGT RISE OR WARNINGS IN THE COCKPIT. I THEN TOOK A QUICK LOOK AT THE ENG AND DIDN'T SEE ANYTHING; SO I DIDN'T SEE ANY PROBS.
