B737-200 FLC FAILS TO DEPLOY FLAPS FOR TKOF. AFTER RECEIVING TKOF WARNING HORN; THE ACFT IS ALLOWED TO CONTINUE THROUGH 70 KTS WHILE FLAPS WERE SET TO TKOF POS AND TKOF WAS CONTINUED.

Narrative

THE TKOF FLAP SETTING WAS NOT SELECTED ON THE BEFORE TKOF CHKLIST. THE ITEM WAS APPARENTLY MISSED DUE TO DISTRS ON TAXI OUT WITH HOLD SHORT INSTRUCTIONS AND OTHER INSTRUCTIONS FROM GND CTL. THE PROB WAS DISCOVERED WHEN THE THROTTLES WERE ADVANCED FOR TKOF AND THE TKOF WARNING HORN SOUNDED. THE FLAPS WERE IMMEDIATELY SELECTED TO 1 DEG AND THE HORN SILENCED IN APPROX 3 SECONDS. SINCE 1 DEG WAS THE PLANNED TKOF FLAP SETTING AND THE ACFT WAS BELOW 70 KTS WHEN THE HORN SILENCED AND THE FLAPS WERE SET; THE TKOF WAS CONTINUED. THE RWY WAS OVER 13000 FT LONG. I THINK A TEST OF THE TKOF WARNING SYS SHOULD BE INCLUDED ON THE BEFORE TKOF CHKLIST. THIS WOULD BE AN EXTRA XCHK OF THE FLAP POS TO HELP AVOID THIS PROB IN THE FUTURE. I BELIEVE A CONTRIBUTING FACTOR IN ADDITION TO THE BUSY GND FREQ WAS FATIGUE OF THE CAPT WHO HAD WORKED 7 OF THE PREVIOUS 8 DAYS AND THIS WAS THE FINAL LEG OF THE TRIP. THE FO HAD ALSO DISCUSSED SOME PERSONAL PROBS THAT MAY HAVE DISTR HIM. SUPPLEMENTAL INFO FROM ACN 353053: NOTED FLAP LEVER NOT IN 1 DEG DETENT BUT OUT OF UP (0 DEG) DETENT.
