Narrative
ACFT #1 ON A MISSED APCH/GAR RWY 12. ACFT #2 DEPARTING RWY 19R. ACFT #1 DID NOT RECEIVE LNDG CLRNC AND EXECUTED GAR. ACFT #2 ROTATED AND SAW THE TFC ON AN INTERCEPT; BOTH ACFT DSNDED AND TURNED L TO AVOID EACH OTHER. THE LCL CTLR WAS COORDINATING A PREVIOUS GAR ON RWY 19R AND WAS DISTR BY FREQ CONGESTION AND TROUBLE COORDINATING ON LANDLINES. ACFT #2 SHOULD NOT HAVE BEEN RELEASED; LCL CTLR WAS NOT AWARE OF THE DC10 ON APCH TO RWY 12; WHICH CALLED 5 TIMES TO GET CLRNC TO LAND BUT TO NO AVAIL. LCL CTLR WAS WAY BEHIND THE PWR CURVE; AND APPARENTLY DID NOT REALIZE THE SIT WAS DEVELOPING. AN ASSISTANT LCL CTLR OR CAB COORDINATOR SHOULD HAVE BEEN ASSISTING THE LCL CTLR. SUPPLEMENTAL INFO FROM ACN 375178: APPEARED BOTH B737 AND DC10 TOOK EVASIVE ACTION AT DEP ENDS OF RWY 19R AND RWY 12. ACFT MISSED BY LESS THAN 1000 FT AFTER A CONSIDERABLE AMOUNT OF MANEUVERING. 2 TWR FREQS IN USE; LCL CTL E 128.42; LCL CTL W 120.1. OPERROR OCCURRED ON FREQ 120.1 (LCL CTL W). LCL CTL W WAS RESPONSIBLE FOR ACFT INVOLVED. I WAS WORKING LCL CTL E.
