A DC9 WAS DISPATCHED WITH THE #2 MAIN TIRE AND WHEEL ASSEMBLY INCORRECTLY INSTALLED.
Narrative
ACR ACFT ARRIVED AT ZZZ ON FLT ABC AT APPROX XA10 AUG/XX/97. ACFT BLEW #2 MAIN TIRE ON LNDG AT ZZZ. ACFT TAXI TO GATE. I; AS A MAINT CREW CHIEF; HAD GATE MECH ON GATE REMOVE AND REPLACE #2 AND #1 MAIN TIRES. #2 TIRE HAD SKID MARK THAT HAD GONE THROUGH ALL LAYERS OF TREAD. MY CREW CHANGED THE TIRES PER DC9 MAINT MANUAL. WE FAILED TO POS THE AXLE NUT SPACER PER THE MAINT MANUAL. THE MECHS WORKING FOR ME WERE NEW MECHS. BOTH HAD LESS THAN A YR TIME WITH ACR. I WAS OVERSEEING THEIR WORK. I FAILED TO FIND THE PROB. WE INSTALLED THE TIRE; TORQUE THE TIRE TO THE RIGHT TORQUE; SAFETY LOCKING SCREWS; INSTALLED HUB CAP. WE THEN REPLACED THE #1 TIRE WITH NO PROBS. WE PERFORMED AN ANTI-SKID CHK AND REPLACED A SKID CTL BOX. FLT WAS DISPATCHED. THE NEXT DAY AFTER SEVERAL LNDGS WHILE ACFT WAS TAXIING OUT FOR FLT; WHEEL NUT CAME LOOSE. THE HUB CAP WAS BROKEN OFF; TIRE WAS LOOSE ON THE AXLE. ACFT WAS TOWED BACK TO GATE WITH TIRE REMOVED. ON INSPECTION; WHEEL NUT AND SPACER HAD COME LOOSE. THE #2 BRAKE WAS REPLACED WITH NEW TIRE ON #2 AND #1 POS. ACFT HAS FLOWN SEVERAL FLTS WITH NO FURTHER PROBS. THE WAY WE WORK AT ACR IN ZZZ IS EACH CREW CHIEF HAS 5-7 GATES WITH 5-7 MECHS ON GATES. I HAVE 7 GATES WITH A MECH ON EACH GATE. THAT NIGHT I WAS VERY BUSY ON ALL GATES WITH NO HELP FROM OTHER CREWS. I WAS WITH THE 2 MECHS ON THE TIRE CHANGE AS MUCH AS POSSIBLE. THEY BOTH HAD NO TRAINING ON DC9 ACFT; JUST WHAT THEY CAN GET WHILE WORKING WITH OTHER MECHS OR WITH ME. I HAVE HAD TRAINING ON DC9 ACFT. I HAVE 31 YRS AT ACR. ALL THE TIME IS WORKING ON LINE POS; INCLUDING 4 YRS AS A SUPVR OVER LINE CREWS.
