A B767-300 WAS DISPATCHED WITH AN ENG THRUST REVERSER DEFERRED BUT RENDERED INOP IMPROPERLY CAUSED BY INCORRECT MEL SPECIAL PROCS.

Narrative

ACFT ARRIVED AT ZZZ ON FLT YY AT GATE X WITH A REVERSE ISLN VLV EICAS INFO MESSAGE. XYZ AND MYSELF DEFERRED THE MESSAGE PER THE MEL; AT THIS POINT THE REVERSER WAS USABLE. UPON TAKING THE LOGBOOK UP TO THE CREW AT ABOUT XA45; THE CAPT REFUSED IT IN THIS CONDITION AND WANTED THE REVERSER INOP. AT THIS POINT I TOLD XYZ THAT I WOULD TAKE CARE OF INOPING THE VALVES IN THE SAIL FAIRING IF HE PUT THE PINS IN THE REVERSER SLEEVES. I ORDERED A PIN FOR THE STOW VALVE; PULLED THE CANNON PLUG ON THE REVERSER ISLN VALVE ON WIRED THE VALVE. AT THAT POINT I RECEIVED THE PIN FOR THE STOW VALVE AND INSTALLED IT AND LOCK WIRED IN POS. WHILE I WAS DOING THIS; FINISHED THE PAPERWORK UP AND RE-RELEASED THE ACFT WITH THE REVERSER INOP. UPON LNDG IN ABC; THE R REVERSER DEPLOYED PART WAY. DURING FURTHER INVESTIGATION; ABC FOUND THAT THE ISLN WAS LOCK WIRED IN THE WRONG POS. I; THE RPTR; WAS THE PERSON THAT LOCK WIRED THAT VALVE. SUPPLEMENTAL INFO FROM ACN 382127: WHILE DEFERRING R REVERSE INOP THE MEL STATED TO MOVE THE ISOLATION VALVE TO THE CLOSED POS. THE VALVE IS NORMALLY CLOSED SO WHEN IT WAS MOVED IT WAS INADVERTENTLY MOVED TO THE ON POS.
