BEECH 1900D ACFT COMMENCED TKOF WITH PARKING BRAKE SET. TKOF WAS ABORTED. FLC RETURNED TO TKOF AGAIN AFTER FINDING THE BRAKE SET. AFTER LNDG A BLOWN TIRE WAS DISCOVERED. FLC WAS REMOVED FROM FLT DUTIES AND PROVIDED EXTRA TRAINING.

Narrative

AFTER APPROX 1 MIN IN POS AND HOLD ON RWY 4L IN BOS; MY CAPT AND I RECEIVED OUR TKOF CLRNC. WE BOTH PERFORMED OUR FINAL ITEMS CHKLISTS AND COMMENCED THE TKOF ROLL. THE ACFT HANDLED AND ACCELERATED NORMALLY UNTIL APPROX V1 SPD WHERE A HESITATION TO ROTATE WAS NOTICED. BOTH CREW MEMBERS SIMULTANEOUSLY CALLED FOR AN ABORT; WHICH WAS ACCOMPLISHED WITHOUT FURTHER INCIDENT. WHILE TAXIING CLR OF THE RWY; THE STEERING AND BRAKING WERE NORMAL. THE REASON FOR THE ABORT WAS RPTED TO ATC AS 'AN ANNUNCIATOR LIGHT' ALONG WITH THE TAIL NUMBER. SHORTLY THEREAFTER WE DISCOVERED THE PARKING BRAKE PARTIALLY SET. AFTER RELEASING THE BRAKE FULLY; WE DEPARTED RWY 4L ENRTE TO PWM. UPON LNDG; THE ACFT HANDLED; STEERED; AND BRAKED NORMALLY ONCE AGAIN. POSTFLT INSPECTION REVEALED A FLAT- SPOTTED #4 MAIN OUTBOARD TIRE. WE CALLED COMPANY DISPATCH AT THAT TIME AND INFORMED DISPATCH AND MAINT OF THE TIRE PROB. WE WERE TOLD THAT THE ABORTED TKOF CONSTITUTED A 'FLT INTERRUPTION' IN BOS; WHICH SHOULD HAVE REQUIRED A PHONE CALL THERE PRIOR TO DEP. WE HAD NOT FOLLOWED OUR COMPANY'S GENERAL OPS MANUAL PROCS ON OBTAINING AN AMENDED RELEASE AFTER A 'MECHANICAL IRREGULARITY.' CAPT AND I WERE REMOVED FROM ACTIVE FLT STATUS AND HAVE RECEIVED ADDITIONAL TRAINING ON THE SUBJECT(S). THE SIT COULD HAVE BEEN AVOIDED BY ENSURING PARKING BRAKE WAS FULLY RELEASED AS REQUIRED IN THE TKOF CHKLIST; OR PERHAPS NOT SETTING IT ON AN ACTIVE RWY. OUR HIGH FREQ OF OP INTO BOS AND POS AND HOLD SCENARIOS LED TO THE CAUSE OF THE INCIDENT. ALTHOUGH THE ABORT WAS HANDLED WELL; WE SHOWED POOR CREW COORD IN THE RPTING OF THE CAUSE AND POOR JUDGEMENT BY NOT CALLING COMPANY AND INSPECTING THE ACFT IN BOS. COMPANY PROCS NOW REQUIRE ALL ABORTS TO HAVE NEW DISPATCH RELEASE WITH INVOLVEMENT OF MAINT DEPT IF NECESSARY.
