LOCATION: CENTRAL WISCONSIN. THIS IS BEING SENT AS A RESULT OF RPT SENT BY CAPT TO CHIEF PLT. CHIEF PLT THEN ENTERED IT AS A COMPANY REPORT AND I WAS JUST NOTIFIED OF THAT FACT. ABOUT 25 MINS IN FLT; #1 FLT ATTENDANT BROUGHT UP A MANILA BOARDMAIL ENVELOPE WITH SOME KIND OF CARGO MANIFEST IN IT. CAPT EXAMINED IT AND NOTICED IT LISTED FORMALDEHYDE. WE WERE CONCERNED THAT WE WERE CARRYING THIS ON BOARD ACFT WITHOUT ANY HAZMAT FORM. WHEN WE GOT TO MSP; CAPT NOTIFIED CARGO AND LCL GND PERSONNEL ABOUT IT. AFTER TKOF TO DFW; MSP CREW CHIEF RADIOED TO US THAT HE DID NOT UNLOAD ANY SUCH CARGO FROM ACFT AND THAT IT MAY HAVE BEEN SHIPPED EARLIER AND THE PAPERWORK WAS LOST AND JUST NOW CATCHING UP TO THE CARGO. SUPPLEMENTAL INFO FROM ACN 403608: I CONTACTED CARGO OPS CTR AND HE IS RESEARCHING THE PROB. HE STATED THAT THE FORMALDEHYDE WAS SUPPOSED TO GO BY TRUCK FROM ORD TO MSP AND THAT IT IS A HAZMAT BECAUSE IT IS CORROSIVE AND FLAMMABLE. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE CAPT RPTR STATED THAT THE HAZMAT WAS NOT FOUND DURING UNLOADING AT HIS DEST AND THE CREW CHIEF ADVISED THAT THE PAPERWORK GOT INADVERTENTLY SEPARATED FROM THE CARGO AND LEFT ABOARD THE ACFT.
