Synopsis

A B767-300 ON TKOF AT 400 FT AGL HAD THE R WING OVERWING ESCAPE SLIDE DEPART THE ACFT.

Narrative

DURING CLBOUT; THE R WING SLIDE WARNING LIGHT ILLUMINATED. WE ACCOMPLISHED THE STEPS IN THE CHKLIST AND CONTACTED OUR LINE MAINT. FLT ATTENDANT RPTS AND THE FO'S VISIT TO THE CABIN INDICATED THAT OUR OVERWING SLIDE WAS NOT ATTACHED TO THE AIRPLANE. A MESSAGE FROM OUR OPS ON THE GND IN ORL CONFIRMED THAT OUR SLIDE WAS FOUND ON THE RWY ENVIRONMENT. ALL SYS IN THE AIRPLANE WERE NORMAL AND WITH THE CONCURRENCE OF OUR LINE MAINT; WE CONTINUED THE FLT TO OUR DEST. I DEEMED IT AS SAFE TO CONTINUE AND LAND AT ORD OR TO RETURN TO ORL. CRUISE; DSCNT; APCH AND LNDG WERE UNEVENTFUL. UPON LNDG; WE FOUND THAT THE R OVERWING SLIDE DOOR HAD OPENED AND THE SLIDE WAS MISSING. ONLY MINOR DAMAGE WAS DISCOVERED ON THE UNPRESSURIZED SKIN AREA AFT OF THE SLIDE DOOR. THE INCIDENT IS UNDER INVESTIGATION TO DETERMINE REASON WHY THE DOOR OPENED INFLT. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE ACFT WAS A NEW B767- 300 DOMESTIC CONFIGN WITH ONLY 20 HRS FLT TIME. THE RPTR SAID NO WORK WAS ACCOMPLISHED ON THE R OVERWING ESCAPE SLIDE AND NOTHING NOTED IN THE ACFT MAINT HISTORY PERTAINING TO THE SLIDE OR DOOR. THE RPTR STATED THE ONLY DAMAGE INCURRED WAS A SMALL DENT IN THE SKIN NEAR THE AFT WHEEL WELL BULKHEAD. THE RPTR SAID NO EICAS WARNING WAS PRESENT PRIOR TO TKOF.
