A FOKKER 100 EXPERIENCED THE SECOND LOSS IN 3 MONTHS OF THE R MAIN LNDG GEAR STRUT DOOR ON THE FIRST FLT AFTER INSTALLATION.
Narrative
ON FEB/XZ/99 AT APPROX YZ00 WE WERE MADE AWARE OF AN INCIDENT INVOLVING ACFT XYZ (F100) INVOLVING THE R-HAND GEAR LEG FOLLOW-UP DOOR THAT WE HAD INSTALLED THE NIGHT BEFORE. THAT NIGHT WE INSTALLED THE DOOR AS PER MY INSTRUCTIONS (AFTER CHKING THE LOGBOOK). AFTER INSTALLING THE DOOR; THE ACFT WAS JACKED AND THE GEAR WAS RAISED AND LOWERED SEVERAL TIMES TO ADJUST THE DOOR (USING THE HYD MULE FOR HYD PRESSURE). AFTER THE ADJUSTMENTS WERE MADE; THE CREW CHIEF CAME TO INSPECT THE INSTALLATION AND ADJUSTMENT OF THE DOOR. THEN THE GEAR WAS RECYCLED AGAIN TO CHK EVERYTHING. ALL THIS WAS DONE IN ACCORDANCE WITH MAINT MANUAL. WHEN SIGNING OFF THE WRITE-UP ON THE LOG; WE NOTICED THAT THE ORIGINAL WRITE-UP WAS 'R STRUT BAY DOOR MISSING;' ENTERED ON NOV/XA/99; MAKING THIS INCIDENT A REPEAT WRITE-UP. THE DOOR WAS INSTALLED ON FEB/XA/99; AND THE INCIDENT WITH ACFT XYZ WAS ON FEB/XB/99. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE DOOR HAD BEEN REPLACED LESS THAN 3 MONTHS EARLIER AND WAS LOST INFLT AND THE CAUSE WAS UNKNOWN. THE RPTR SAID THE DOOR WAS REPLACED BY THE MAINT MANUAL PROCS AND WAS TESTED BY RETRACTING THE GEAR NUMEROUS TIMES TO CHK FIT AND CLRNC. THE RPTR SAID IT'S VERY POSSIBLE THE RIGGING AND ADJUSTMENT PROC IS WRONG. THE RPTR STATED IT MAY WELL BE THE HYD CART USED FOR THE GEAR RETRACTION COULD NOT SUPPLY THE CORRECT PRESSURE AND VOLUME TO FULLY RETRACT THE GEAR FOR ADJUSTMENT. THE RPTR SAID NO INFO HAS BEEN AVAILABLE AS TO THE DIRECT CAUSE OF THE LOSS OF THESE DOORS OR ANY CORRECTIVE ACTION.
