A BOEING 767-300 HAD AN ENG FAILURE ON TKOF FROM TLV; FO.

Narrative

R ENG FAILED ON TKOF AT VR. DECLARED EMER AND RETURNED TO TLV. LANDED OVERWT FOR SAFETY. ALL 3 CREW MEMBERS USED TO COMPLETE ALL RELEVANT CHKLISTS. SUPPLEMENTAL INFO FROM ACN 446069: SHORTLY AFTER ROTATION; HEARD A LOUD 'BANG;' SAW A FLASH OF ORANGE LIGHT OUT R WINDOWS AND FELT STRONG VIBRATIONS. I THEN NOTICED R EGT GAUGE INDICATE RED AND OTHER INDICATIONS OF SURGING ENG/COMPRESSOR STALL. WE FLEW ENG-OUT PROC STRAIGHT-OUT AND CAPT DECLARED AN EMER WITH TWR; AND REQUESTED IMMEDIATE RETURN TO ARPT WITH EQUIP STANDING BY FOR US. WE WERE CLRED TO LAND ON ANY RWY. AFTER COMPLETION OF ENG FAILURE; ENG INOP APCH; DSCNT/APCH AND LNDG CHKLISTS; WE BRIEFED THE EVAC CHKLIST. FLT SVC MGR WAS KEPT IN LOOP THE WHOLE TIME AND KEPT FLT ATTENDANTS (AND PAX) WELL INFORMED AND PREPARED. WE LANDED OVERWT; WITH A HIGH APCH SPD. TOUCHDOWN WAS VERY SMOOTH AND WE ROLLED TO END OF RWY AND TURNED OFF ONTO ANOTHER RWY. EQUIP WAS WAITING AND INFORMED US THAT R ENG HAD LARGE HOLE BLOWN THROUGH INBOARD SIDE; FAN BLADES DESTROYED AND OUR BRAKES WERE ORANGE; BUT NO SMOKE OR FIRE. AFTER STOPPING; 7 OF 8 MAIN TIRES WENT FLAT AS FUSE PLUGS BLEW. I SUSPECT FOREIGN OBJECT DAMAGE WAS RESPONSIBLE.
