Narrative
ON A FLT FROM LAS TO BUR ON OCT/TUE/99; WHILE FLYING AT 10000 FT; 12 MI FROM DAG IN CAVU CONDITIONS WITHIN THE IFR SYS; I EXPERIENCED AN ENG FAILURE. THE ENG HAD JUST RECENTLY BEEN OVERHAULED WITH NEW TYPE CYLINDERS UNDER WARRANTY ON JUL/WED/99; WITH APPROX 48 HRS SINCE THE WORK WAS COMPLETED. THE EVENT ENDED WITH A SAFE NO PWR LNDG; OTHER THAN A VERY SERIOUS QUESTION ON HOW SUCH A FAILURE OCCURRED. THE WORK HAD BEEN DONE BY ABC ENGS INC. FAA REPAIR STATION WXXX OF ZZZ. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE ACFT WAS LANDED WITH NO PWR AT DAGGETT ARPT. THE RPTR SAID THE ENG TEAR DOWN WAS ACCOMPLISHED AT THE REPAIR STATION AND THE RESULTS ARE NOT CONCLUSIVE. THE RPTR SAID THE SHOP THAT REPLACED THE PISTONS AND CYLINDERS CLAIM THE SHOP THAT OVERHAULED THE ENG 1200 HRS PREVIOUS HAD INSTALLED DEFECTIVE COUNTER WTS ON THE CRANKSHAFT. THE RPTR STATED THE OVERHAUL REPAIR SHOP MAKES THE CLAIM THAT #1 PISTON WAS DEFECTIVE AND FAILED AFTER 48 FLT HRS. THE RPTR SAID HE WAS HOPING THE FAA WOULD INVESTIGATE AND DETERMINE THE COMPONENT FAILURE BUT ONLY RECEIVED A LETTER STATING THE ENG WAS ASSEMBLED USING THE CORRECT PROCS NOT ADDRESSING THE COMPONENT FAILURE ISSUE.
