AN MD11 WAS OPERATED IN NON COMPLIANCE WITH OUT-OF-LIMITS DAMAGE TO THE L INBOARD FLAP UPPER SURFACE CAUSED BY A THROWN TIRE TREAD.

Narrative

ACFT ARRIVED IN ZZZ; FO; AFTER THROWING A RECAP TIRE ON DEP FROM PDX; OR. MAINT REPLACED TIRE AND WHEEL AND 2 HYD LINES THAT WERE DAMAGED WHEN THE RECAP CAME OFF. ALSO; THE ANTISKID WAS INOP AND DEFERRED PER MEL. AFTER LOGBOOK WAS SIGNED OFF AND MAINT ITEMS CLRED; FO MADE A PREFLT INSPECTION OF EXTERIOR OF ACFT AND WE DEPARTED FOR PDX. AFTER ARR IN PDX; MAINT DISCOVERED DAMAGE TO INBOARD FLAP PANEL THAT APPARENTLY WAS OVERLOOKED BY MAINT IN ZZZ AND ALSO BY FO ON EXTERIOR PREFLT INSPECTION. ACFT WAS FERRIED TO ZYZ FOR REPAIRS. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE ACFT HAD THROWN TIRE TREAD DAMAGE TO THE GEAR; FLAPS AND HYD SYS ON ARR AT ZZZ; FO. THE RPTR SAID THE DAMAGE OCCURRED ON TKOF FROM ZZZ; US; AND THE TWR ADVISED THE FLT OF TIRE DEBRIS ON THE RWY. THE RPTR SAID THE TECHNICIANS MADE THE THROWN TREAD DAMAGE CHK PER THE MAINT MANUAL AND REPAIRED THE DAMAGED HYD LINES; REPLACED THE TIRE AND DEFERRED THE ANTISKID SYS. THE RPTR STATED THE FO DID NOT RPT ANY ADDITIONAL DAMAGE ON THE PREFLT WALKAROUND CHK. THE RPTR SAID ON ARR IN ZZZ; US; THE L INBOARD TRAILING EDGE FLAP WAS THOROUGHLY CHKED AND UPPER SURFACE DAMAGE WAS DISCOVERED. THE RPTR STATED THE DAMAGE WAS OUT OF LIMITS OF THE STRUCTURAL REPAIR MANUAL AND MAINT FERRIED FOR REPAIR. SUPPLEMENTAL INFO FROM ACN 452458: I CONSULTED THE MAINT MANUAL AND COULD NOT FIND ANYTHING THAT INSTRUCTED ME TO LOWER THE FLAPS TO PERFORM THE INSPECTION. AFTER; THE RECAP OF THE TIRE HAD SEPARATED OR MIGRATED.
