Synopsis

A B757 CAPT; ON THE ANPU 2 DEP FROM RCTP; INTERCEPTED THE WRONG RADIAL; CAUSING TAIPEI TO ISSUE VECTORS AROUND OTHER TFC.

Narrative

OUR FLT WAS FILED DIRECT TO ANPU A-1 VENUS RJAA. THE CLRNC WAS ANPU 2; KIKIT TRANSITION; FLT PLAN RTE; ETC. WE DEPARTED RWY 6 FROM TAIPEI AND AT 17 MI ON THE SID MADE A R TURN TO 080 DEGS AND MISTAKENLY INTERCEPTED THE AIRWAY ON A-1 (OUR FILED RTE) INSTEAD OF A R TURN TO 090 DEGS TO INTERCEPT ANPU 072 DEG RADIAL; OUR CLRNC FOR THE KIKIT TRANSITION. WE INTERCEPTED THE ANPU 052 DEG RADIAL (A-1) AT 10 DME INSTEAD OF THE ANPU 072 DEG RADIAL. AT THE TIME OF INTERCEPT WE WERE 6 MI FROM THE ANPU 072 DEG RADIAL. WE HAD ONLY ESTABLISHED TRACK WHEN TAIPEI DEP ASKED OUR HDG. WE ADVISED 050 DEGS. TAIPEI DEP ASKED WHICH DEP WE WERE ON. WE ADVISED THE KIKIT. REALIZING OUR ERROR; WE IMMEDIATELY INITIATED A R TURN TO INTERCEPT THE ANPU 072 DEG RADIAL. WHILE WE WERE IN THE TURN; TAIPEI ISSUED VECTORS OF 090 DEGS TO AVOID TFC. CONTRIBUTING FACTORS: THE CREW WAS VERY TIRED FROM A MINIMUM OVERNIGHT REST. THE CAPT INDICATED HE HAD ONLY SLEPT 4-5 HRS. THE FO HAD COMPLETED 5 TRANSPACIFIC FLTS IN A ROW. THE ANPU 2 DEP IS A COMPLEX; 2-PAGE FOLD-OUT THAT IS VERY CLUTTERED AND CONFUSING WITH MULTIPLE XOVERS. THE SCHEMATIC WAS SIMPLY MISREAD. THE CREW HAS FLOWN THE ANPU 2 KIKIT TRANSITION MANY TIMES. CORRECTIVE ACTION: CREATE 3 OR MORE DEPS ON SEPARATE PAGES: ANPU 1; ANPU 2; ANPU 3; ETC; WITH THE RESPECTIVE TRANSITIONS APPLICABLE TO A SPECIFIC DEP. AVOID ALL THE XOVERS AND CLUTTER WITH THE TROUBLESOME SCHEMATIC WITH THE ANPU 2.
