Narrative
ON FEB/FRI/00; XYZ WAS DUE INTO ZZZ ARPT ON TRIP #XXX AT APPROX XA55 HRS. IT WAS DUE OUT AT XA55 HRS. IT CAME IN WITH A PIREP; ACFT R PACK TRIP OFF LIGHT CAME ON; SELECTED A WARMER TEMP AND LIGHT CAME ON AGAIN. ON THE OVERNIGHT SVC CHK THE MECH WXVX EMPLOYEE VXVX FOUND THAT L-HAND NOSEWHEEL STEERING ACTUATOR LEAKING AND OUT OF MAINT MANUAL LIMITS AND WROTE IT IN THE LOGBOOK. I CALLED LINE CTL AT ZZZ AND ASKED THEM TO SEE IF THE ACFT CAN LEAVE ZZZ WITH THE R-HAND AIR CONDITIONING PACK INOP AT XH55. ACR MAINT CTL SAID NO. NOW WE HAVE 2 PROBS; A R AIR CONDITIONING PACK AND POSSIBLE R BLEED AIR (ENG) INOP AND A LEAKING L-HAND NOSE GEAR STEERING ACTUATOR THAT IS OUT OF LIMITS; BTWN XB30 AND XC30 THE SHIFT MGR; WWWZ EMPLOYEE WYWY AND MY IMMEDIATE SUPVR ABC ZYZY; EMPLOYEE OWYO; WERE FULLY AWARE AND NOTIFIED BY ME. THIS ACFT IT HAS CHRONIC HISTORY; TECHNICAL SUPPORT HAD ALREADY BEEN INVOLVED AND SUSPECTED THE HIGH STAGE VALVE ON THE R-HAND ENG MIGHT BE STAYING PARTIALLY OPEN CAUSING HOT AIR TO TRIP THE R-HAND PACK. WITH NO DIRECTION FROM ABC ZYZY MYSELF; WXVX; AND ANOTHER MECH CBA DYW; EMPLOYEE WYD WENT TO WWWZ AT XD45 AND TOLD HIM OF THE PLAN WE HAD. IT WAS TO CHANGE THE 390 DEG AND 490 DEG SENSORS AT THE R-HAND ENG PYLON AND NON-ROUTINE ITEMS (SERIALIZED) WERE GENERATED. ABC ZYZY ALSO CAME INTO THE SHIFT MGR'S OFFICE AND AT XG00; ME; ABC ZYZY; WXVX AND CBA DYW ALL AGREED THAT THE 390 DEG AND 490 DEG ENG BLEED SENSORS WOULD BE CHANGED AND A CROSSOVER TEST WOULD BE PERFORMED AFTER THE SENSORS WERE CHANGED WRITE-UP BYX. NON-ROUTINE CARD TO CHANGE THE SENSORS IS ALSO MISSING; REFING THE ATTACHED COPY FROM SR MGR MXX CONCERNING ME AND MY DOCUMENTATION OF THE EVENT PER GMM. HE ALSO RECOMMENDED I FILL OUT THIS NASA FORM. THANK YOU VERY MUCH.
