B747-200F HAS ENG FAILURE ON TKOF FOLLOWED BY FIRE WARNING AND SUCCESSFUL RETURN TO LAND. EVAC COMPLETED WITHOUT INCIDENT.

Narrative

WE DEPARTED GUAYAQUIL; ECUADOR; AT XB00Z BOUND FOR MIAMI; FL; PUSHED BACK AT XA40 TAXI TO RWY 21 FOR MAX PWR/NO PACKS TKOF. AT XB00 THE CREW HAD POS THE ACFT ONTO RWY 21 FOR TKOF. ALL CHKLISTS WERE COMPLETED AND THE CAPT/FE WERE MAKING THE ENG RUN-UP JUST PRIOR TO TKOF. AFTER BRAKES WERE RELEASED; THE FO TOOK CTL OF ACFT FOR TKOF (AS PREVIOUSLY BRIEFED). ACFT ACCELERATED NORMALLY TO TKOF SPD; TOOK OFF AS PLANNED; THEN AT 400 FT WHILE THE GEAR WAS RETRACTING THE CREW FELT A VIBRATION FOLLOWED BY YAW TO THE L AND 2 LOUD EXPLOSIONS. ALL INDICATIONS POINTED TO AN ENG FAILURE; BUT WE GOT THE FIRE BELL AND ALL LIGHTS IN THE COCKPIT SHORTLY AFTER THAT INDICATING THAT AN ENG FIRE HAD OCCURRED. THEN WE FOLLOWED THE COMPANY PROC AND TOOK THE AIRPLANE TO AN ALT OF 817 FT AGL AND COMMENCED THE ENG FIRE CHKLIST AT THE SAME TIME THE CAPT DECLARED THE EMER AND INFORMED THE CTL TWR THAT NATURE OF THE EMER. THE FIRE TRUCKS WERE CALLED OUT TO THE RWY AND REMAINED ON STANDBY. AFTER THE ENG FIRE CHKLIST WAS COMPLETED THE CAPT COMMANDED THE FE TO BEGIN DUMPING FUEL. AT THAT POINT IT WAS ALSO DISCOVERED THAT THE #1 HYD HAD BEEN LOST. THE HYD CHKLIST WAS THEN INITIATED. INBOARD FLAPS AND GEAR HAD TO BE ALTERNATELY EXTENDED. THE FO; ON THE CAPT'S COMMAND; WAS POSITIONING THE ACFT ON FINAL APCH TO LAND AT XB00Z. WE LANDED SAFELY IN GUAYAQUIL ARPT WITH NO INJURIES OR FATALITIES TO ANY OCCUPANTS OR CREW MEMBERS; THEN CAPT INSTRUCTED THE FE TO COMMENCE THE EVAC CHKLIST. THE 7 OCCUPANTS INCLUDING 3 CREW MEMBERS WERE EVACED WHILE THE FIRE DEPT TOOK CTL OF THE ACFT. THE TWR ASKED THE CREW TO TAXI OFF THE ACTIVE RWY; BUT WE WERE UNABLE SINCE THERE WAS NOT ANY NOSE GEAR STEERING. 30 MINS AFTER LNDG; THE ACFT WAS REMOVED FROM RWY BY A TOW MOVER UNDER THE SUPERVISION OF A LINE MECH. WHEN THE ACFT PARKED AT THE GATE; THE CREW BOARDED THE ACFT AND BEGAN TO ENTER ITEMS IN THE ACFT MAINT BOOK AND COMPLETE ALL THE APPROPRIATE PAPERWORK. SINCE THE FLT HAD BEEN TERMINATED THE SECURE COCKPIT CHKLIST WAS COMPLETED.
