A C421C ON TKOF ROLL HAD THE NOSE GEAR COLLAPSE WITH PROPS STRIKING THE RWY INCURRING DAMAGE TO THE ACFT.

Narrative

I STARTED FLT ON JUN/XA/00 AND HEARD WARNING HORN WHICH I TOOK TO BE STALL WARNING HORN. I SHUT DOWN THE ENGS AND INSPECTED STALL SWITCH AND GEAR (3 GREEN AND NO GEAR UNSAFE CONDITIONS) AND HYD FLUID. CREW MEMBER ADVISED ME THAT THIS HAD HAPPENED EARLIER AND DISCONTINUED ONCE AIRBORNE. I RESTARTED ENGS. ON TKOF AT VR; NOSEWHEEL COLLAPSED AND I STRUCK BOTH PROPS ON RWY BEFORE BECOMING AIRBORNE. I ELECTED TO PROCEED FROM ZZZ TO XXX. LOST COM (PIECE OF PROP SEVERED WIRES IN FUSELAGE). MADE STRAIGHT-IN TO RWY 19R; SHUT DOWN ENGS. UNLOADED. TOWED ACFT TO PARKING. MAINT PERSONNEL CANNOT; AS OF SECOND DAY AFTER THE INCIDENT; DUPLICATE THE PROB. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED NO POSITIVE CAUSE OF THE NOSE GEAR COLLAPSE HAS BEEN DETERMINED. THE RPTR SAID THE NOSE GEAR ACTUATOR WAS REMOVED AND SENT TO THE MANUFACTURER FOR TEARDOWN AND NO FAULT WAS FOUND. THE RPTR STATED THE AIRPLANE HAS NOT FLOWN SINCE THE INCIDENT AS THE ENGS ARE BEING DISASSEMBLED AND WON'T BE READY FOR SOME TIME. THE RPTR SAID THE FAA HAS CLASSIFIED THE EVENT AS AN INCIDENT.
