Synopsis

A FOKKER 100 IN CRUISE AT FL180 DECLARED AN EMER AND DIVERTED DUE TO #1 ENG THRUST REVERSER DEPLOYED INFLT CAUSED BY ELECTRICAL AND MECHANICAL FAILURE.

Narrative

DURING DSCNT FROM CRUISE; WE RECEIVED A MASTER CAUTION DOUBLE-CHIME; AND AN AMBER (REVERSE ENG #1) MESSAGE. THIS WAS ACCOMPANIED BY A L YAW AND A MODERATE TO SEVERE BUFFET. THE MFDU CHKLIST AND ALL QRH PROCS WERE COMPLETED. AN EMER WAS DECLARED; AND WE DIVERTED TO ORF. THE FLT ATTENDANTS WERE DIRECTED TO PREPARE THE CABIN FOR AN EMER LNDG. LNDG IN ORF WAS COMPLETED UNEVENTFULLY. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THERE WAS NO PRIOR WARNING JUST THE MASTER CAUTION WITH AMBER #1 REVERSE LIGHT AND A HARD L YAW. THE RPTR SAID THE VIBRATION SHOOK AND DUMPED DISHES IN THE AFT CABIN. THE RPTR STATED AFTER THE ENG WAS SHUT DOWN AND SPD WAS REDUCED THE VIBRATION WAS LESS AND THE AIRPLANE WAS MANAGEABLE. THE RPTR SAID MAINT ADVISED 3 FAILURES WERE FOUND; A LNDG GEAR SWITCH FAILED TO THE GND POS; A THROTTLE QUADRANT SWITCH IN THE PEDESTAL FAILED AND AN UNKNOWN MECHANICAL FAILURE ON THE THRUST REVERSER. THE RPTR STATED THE NTSB AND THE FAA WERE ADVISED OF THIS INCIDENT.
