A B767-200 DURING A L ENG HIGH PWR RUN-UP FOR AN OIL SEAL LEAK CHK HAD A CATASTROPHIC HIGH PRESSURE TURBINE DISC FAILURE INCURRING MAJOR DAMAGE TO THE ENG AND ACFT. SEE ALSO ACN 487160.

Narrative

DURING ENG RUN-UP FOR OIL CONSUMPTION CHK; L ENG SUFFERED A CATASTROPHIC HIGH PRESSURE/TURBINE DISC FAILURE. L ENG TOTAL LOSS; ACFT L WING AND L GEAR SEVERELY DAMAGED DUE TO FIRE. R ENG DAMAGED FROM DEBRIS. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE ACFT MAINT FERRIED FROM AN OVERSEAS STATION INTO ZZZ TO CORRECT A CHRONIC OIL LEAK ON #1 ENG. THE RPTR SAID AFTER INSTALLATION OF A GEARBOX CARBON SEAL ON THE INTEGRATED DRIVE GENERATOR IT WAS DECIDED TO RUN THE ENG AT HIGH PWR TO LEAK CHK THE SEAL. THE RPTR SAID THE ENG WAS TAKEN TO THE RUN PAD AND AT 88% N1 SPD; THE HIGH PRESSURE TURBINE DISC FAILED. THE RPTR STATED THE ACFT JUMPED 3 FT IN THE AIR AND TWISTED SIDEWAYS. THE RPTR SAID THE DISC FAILED IN 3 PIECES; ONE WENT OUT THE L REVERSER COWLING CAUSING EXTENSIVE DAMAGE AND WAS FOUND 1500 FT FROM THE ENG. THE RPTR STATED THE SECOND WENT THROUGH THE COWLING DIRECTLY INTO THE PAVEMENT TO DEPTH OF 12 INCHES. THE RPTR SAID THE THIRD PIECE EXITED THE ENG ABOUT THE 2 O'CLOCK POS; SEVERELY DAMAGING THE PYLON; CUTTING THE FORWARD WING SPAR AND PUNCTURING THE L WING CTR TANK BULKHEAD. THE RPTR STATED THE FIREWALL HANDLE WAS PULLED AND THE FIRE BOTTLES DISCHARGED; BUT WITH NO COWLING TO CONTAIN THE AGENT; IT WAS HOPELESS. THE RPTR SAID THE TWR WAS ALERTED TO THE EMER AND THE R ENG AND APU FIREWALL HANDLES WERE PULLED. THE RPTR STATED 3 TECHNICIANS WERE ONBOARD AND AFTER SECURING THE AIRPLANE THEY EXITED FROM AN AFT ENTRY ESCAPE SLIDE. THE RPTR SAID EXTENSIVE DAMAGE WAS CAUSED TO THE LNDG GEAR AND LOWER L WING SURFACE BY THE FIRE. THE RPTR STATED THE FIRE TRUCKS RESPONDING USED 3 TRUCK LOADS OF FOAM AND REQUIRED CITY TRUCKS TO CTL THE FIRE.
