Synopsis

A DASH 8-200 DURING A MAIN FUEL CTL CHANGE WAS FOUND TO HAVE A LEAKING FUEL NOZZLE FLOW DIVIDER 'B' NUT LEAKING CAUSED BY NOT BEING SAFETY WIRED.

Narrative

ON NOV/XA/00; ACR ACFT XYZ ARRIVED IN THE ZZZ ACR MAINT BASE FOR ITS #1 ENG TO BE CHANGED. I ARRIVED ON THE MORNING OF NOV/SAT/00 AS THE FINAL INSTALLATION WERE BEING PERFORMED. AFTER THE MECHS WERE DONE; I INSPECTED THE ENG FOR ANY DISCREPANCIES. I DIDN'T NOTICE ANY MAJOR PROBS AT THAT TIME. AT THAT POINT WE RAN THE PLANE FOR ITS FIRST LEAK CHK AFTER THE NEW ENG WAS INSTALLED. THAT LEAK CHK WENT WELL WITH NO BIG PROBS. THEN WE RAN IT AGAIN FOR THE POST ENG PWR ASSURANCE CHK. THAT LEAK CHK WE DISCOVERED A LEAKING FUEL LINE UNDER THE MECHANICAL FUEL CTL UNIT. THE LINE WAS TIGHTENED AND A THIRD RUNUP AND LEAK CHK WAS PERFORMED SHOWING NO FURTHER PROBS. THE ACFT WAS RETURNED TO SVC AND IT HAD GONE UNNOTICED THAT THE NEWLY INSTALLED FUEL NOZZLE FLOW DIVIDER B NUT HAD NOT BEEN SAFETY WIRED. IT HAD NOT LEAKED DURING OUR LEAK CHKS. THE PLANE FLEW ON NOV/SUN/00. IT FLEW FOR 2 LEGS BEFORE IT HAD A PROB. DURING ITS THIRD LEG; THE PLT SAID THE #1 ENG SHUT DOWN. 2 MECHS WENT TO MEET THE PLANE AND THEY CHANGED THE MECHANICAL FUEL CTL AND DISCOVERED THERE WAS FUEL LEAKING FROM THE FLOW DIVIDER. THE PLANE FLEW TO NORFOLK AFTER THE LINE MAINT WAS PERFORMED AND THE FUEL FLOW DIVIDER WAS TAKEN OFF AND INSPECTED. IT WAS DISCOVERED THAT THE O-RING WAS CUT.
