A FOKKER 100 WHILE BEING DEICED EXPERIENCED AN UNCONTAINED APU TURBINE FAILURE INCURRING DAMAGE TO THE APU ENCLOSURE AND ACFT BULKHEAD.
Narrative
ON MAR/XA/01; I WAS CALLED TO DO A FIELD TRIP TO ZZZ ON ACFT XYZ WHICH WAS DAMAGED BY APU FAILURE. UPON ARRIVING AT THE ACFT; SUPVR X INSTRUCTED ME TO REMOVE WHAT REMAINED OF THE APU AND TIRE BOX TO INSPECT FOR FURTHER DAMAGE AFTER THE STRUCTURES CREWS FINISHED THE TEMPORARY REPAIR. I WAS INSTRUCTED TO REINSTALL THE TIRE BOX AND APU FOR FERRY FLT THE NEXT DAY. WHEN WE ARRIVED; THE SUPVR REQUESTED A BILL OF WORK THAT DIDN'T ARRIVE. I THOUGHT THAT ALL THE PAPERWORK WAS BEING HANDLED BY SUPVR X AS LOGBOOK ENTRIES. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE APU TURBINE FAILURE CAUSED HVY DAMAGE TO THE APU ENCLOSURE AND DAMAGED AND PUNCTURED THE BULKHEAD FORWARD OF THE APU. THE RPTR SAID THE REPAIR CREW WAS TO MAKE INTERIM REPAIRS TO MAKE THE AIRPLANE READY FOR A MAINT FERRY. THE RPTR STATED THE PAPERWORK WAS POORLY MANAGED WITH NOT ALL JOBS DESCRIBED AND SIGNED OFF AFTER ACCOMPLISHMENT. THE RPTR SAID THERE WERE NO FERRY PERMITS OBTAINED FROM THE COUNTRY WHERE THE INCIDENT OCCURRED AND THE AUTH OF THE MAINT SUPVR TO ISSUE A MAINT FERRY PERMIT HAD EXPIRED. CALLBACK CONVERSATION WITH RPTR ACN 503714 REVEALED THE FOLLOWING INFO: THE RPTR STATED THE AIRPLANE WAS ON THE GND BEING DEICED WHEN THE APU EXPLODED AND ONLY FELT A BUMP LIKE A TRUCK STRUCK THE AIRPLANE. THE RPTR SAID THE #2 FLT ATTENDANT WAS SEATED IN THE JUMP SEAT ON THE AFT BULKHEAD AND RAN FORWARD TO ALERT THE FLC. THE RPTR STATED THE AFT FLT ATTENDANT DESCRIBED THE INCIDENT AS BEING REAR ENDED IN AN AUTOMOBILE. THE RPTR SAID NO INJURIES WERE INCURRED AND THE AIRPLANE WAS NOT EVACED.
