Narrative

THE PURPOSE OF THE FLT WAS TO CONDUCT A LINE ORIENTED FLT TRAINING (LOFT). THIS LOFT FLT WAS TO BE THE FINAL PORTION OF AN INITIAL EQUIP/QUALIFICATION TRAINING OF A NEW BELL 222U CAPT. THE PLANNED FLT WAS TO INCLUDE A VFR DEP FROM A BUILDING. WE PICKED UP OUR CLRNC TO VUJ. UPON COMPLETION OF THIS LEG; WE INTENDED TO PICK UP A SECOND IFR CLRNC TO EQY. CLT APCH CLRED US TO VUJ DIRECT; MAINTAIN 5000 FT. JUST SHORT OF VUJ (1-3 NM); WE WERE HANDED OFF TO GREENSBORO APCH. I REQUESTED RADAR VECTORS TO THE ILS RWY 22L AT VUJ. GSO WAS BUSY AND THEY INDICATED SO. I ASKED IF THEY WANTED US TO HOLD AT ALLER LOM (VU). THEY RESPONDED WITH A CLRNC TO FLY HDG 090 DEGS; MAINTAIN 5000 FT. LATER WE WERE GIVEN A L TURN TO BASE FOR THE ILS. WE WERE ALSO INFORMED OF A LEVEL 3 RADAR RETURN ON FINAL. I ELECTED TO ABANDON THE APCH AND REQUESTED CLRNC TO EQY. WE WERE GIVEN A 200 DEG HDG AND TOLD TO MAINTAIN 4000 FT; INTERCEPT V409. THE PLT BEING TRAINED; TOOK OUT THE IFR ENRTE CHART TO LOCATE V409. HE THEN SET THE NAV PROB UP ON THE HSI AND NAV PACKAGE. ONCE HE WAS DONE; WE WERE TOLD TO CONTACT CLT APCH. WHILE THE PLT BEING TRAINED COMPLETED SETTING UP THE NAV PROB; I REVIEWED THE CHART. I SAW IT WAS SET UP INCORRECTLY. I POINTED THIS OUT TO THE PLT BEING TRAINED AND HE CORRECTED THE SIT. WE HAD FLOWN THROUGH F409. WHAT HAPPENED WAS THE PLT BEING TRAINED TOOK A QUICK GLANCE AT THE CHART AND SAW V409 EXTENDING E OF THE CLT VOR ON THE 093 DEG RADIAL. HE FAILED TO SEE IT; TURNED NE AT LOCUS INTXN VIA THE 231 DEG RADIAL TO LIBERTY VOR (LIB). HE THOUGHT IT CONTINUED E TO SANDHILL VOR (SDZ). BY HIS INTERP; HE NEEDED TO CONTINUE ON THE ASSIGNED 200 DEG HDG TO INTERCEPT V409 E OF LOCAS INTXN (THIS; OF COURSE; DOES NOT EXIST). WHEN I CAUGHT THE MISTAKE; WE WERE S OF V409. IF I HAD HAD 2 SETS OF CHARTS; I COULD HAVE FOLLOWED ALONG AS THE PLT SET UP.
