ARPT VEHICLE OPERATOR ADVISES 'CLR OF RWY' AFTER BGR CTLR CLRS ACR FOR TKOF AND IS AIRBORNE.

Narrative

I WAS THE LCL CTLR IN THE CAB. IT WAS VERY SLOW TFC-WISE. I HAD WORKED LCL XA00-XB30; GND FROM XC45-XD50+/-; AND NOW WAS BACK ON LCL RIGHT AFTER LUNCH. I HAD BEEN ON POS ABOUT 45 MINS WHEN MR X; AN ARPT DUTY OFFICER; REQUESTED TO PROCEED ONTO THE RWY FOR A RWY INSPECTION. ABOUT 10-15 MINS LATER AN SF34; SAAB REGIONAL AIRLINE TAXIED OUT TO DEPART FROM AN INTXN. I CLRED HIM FOR TKOF AND HE DEPARTED. I SWITCHED HIM TO DEP OVER DEP END. A FEW MINS LATER MR X CALLED CLR OF THE RWY AT THE DEP END. I DO NOT KNOW IF HE WAS STILL ON THE RWY OR NOT. I THINK HE WAS OFF BUT HAD NOT CALLED CLR. A LOT OF TIME HAD GONE BY. THE PLT NEVER SAID A WORD ABOUT THE VEHICLE AND MR X NEVER SAID ANYTHING; THAT'S WHY I DON'T KNOW THE ESTIMATED MISS -- WAS HE CLR? I DID NOT TURN OUR 'VEHICLE ON RWY SIGN;' OR VEHICLE ON RWY FLASH' ON THE BRITE. I WAS NOT PAYING AS CLOSE ATTN AS I SHOULD HAVE. THERE WERE TWO OTHERS IN THE CAB; ONE ON BREAK AND THE OTHER ON GND. THEY WOULD HAVE SPOKEN UP I THINK IF THEY SAW THIS OCCUR. MAYBE I CLRED THE VEHICLE ON THE RWY LAST; AFTER SF34 TOOK OFF; BUT I THINK THE VEHICLE HAD CLRED AND NOT CALLED. FACTORS AFFECTING PERFORMANCE: I WAS VERY; VERY TIRED. I WAS OUT SICK YESTERDAY AND SHOULD HAVE BEEN TODAY ON SICK LEAVE; BUT CHOSE TO COME IN. I SIMPLY DIDN'T PAY ATTN. I WILL IN THE FUTURE WRITE DOWN THE TIME ANY VEHICLE GOES ONTO THE ACTIVE AND WRITE DOWN THE TIME OFF THE RWY. LIKE I SAID EARLIER; I AM NOT SURE A DEAL OCCURRED; BUT NEED TO PAY MORE ATTN. PROBS IN PERSONAL LIFE AFFECT OUR CONCENTRATION. I WAS OUT ON SICK LEAVE DUE TO STRESS AND PROBS AT HOME. I WAS NOT STRESSED AT THIS TIME; ONLY VERY; VERY; VERY TIRED.
